- QUOTES DUE BY FRIDAY, JANUARY 13, 5 PM PST. ***SITE VISIT - 10 AM ON WEDNESDAY, JANUARY 11.*** This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is AG-02RC-S-17-0011 and is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-91. This is a Total Small Business Set-Aside and the NAICS Code is 561720, Size Standard is $18M. The provision at 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition and a statement regarding any addenda to the provision. EVALUATION CRITERIA: The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers. 1. Relevant Past Performance for Janitorial services for past 3 years. 2. List personnel assigned to perform janitorial services at the work site, and their experience to meet the Government Requirements. 3. Price. 4. Quality Control Plan. (see 1.5 on pages 3-4) QUOTERS MUST SUBMIT THE FOLLOWING: 1. Schedule of Items with Pricing 2. Experience Questionnaire Form - Attachment. List personnel assigned to perform janitorial services at the work site, and their experience to meet the Government Requirements 3. Quality Control Plan. (see 1.5 on pages 3-4) 4. State of Idaho Certificate of Verification of Workers Compensation Insurance Form - only after award of contract Quoters must include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items. The clause at 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition and a statement regarding any addenda to the clause. The clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items, applies to this acquisition.
