Description
The Fayetteville, NC VA Medical Center (FNC) has a requirement for Ambulance Transportation Services. The contractor shall provide all labor, material, equipment, vehicles, vehicle maintenance, documentation, reports, medical personnel and supervision necessary to provide Ambulance services for beneficiaries of the FNC and surrounding Community Based Outpatient Clinics (CBOC). The FNC serves the following counties: Bladen, Brunswick, Columbus, Cumberland, Duplin. Harnett, Hoke, Jones, Lee, Lenoir, Moore, New Hanover, Onslow, Pender, Richmond, Robeson, Sampson, Scotland, and Wayne, NC and Dillon and Marlboro counties in S.C. Contractor shall furnish vehicles sufficient to provide full and complete coverage for pick-up and/or delivery of all veteran patients requiring transport by Ambulance. This is a sources sought for the DCVAMC to determine the availability of potential sources (and their size classification relative to NAICS 621910 (Ambulance Services) having the skills and capabilities necessary to provide services in accordance with the attached SOW. All interested vendors are invited to provide information to contribute to this market survey/sources sought including, commercial market information. This is not a solicitation announcement. No award will be made from this sources sought. Requested information should be submitted by email to mark.edzards@va.gov by 3:00pm EST January 6, 2017. Provide only the requested information below. REQUESTED INFORMATION STATEMENT OF CABABILITY: Submit a brief [ Five (5) pages or less] capability statement explaining your capability to perform ambulance transportation services for the DCVAMC. Include past experience in performing these services for the VA, other Government (Federal or State) agency, or for a private medical facility. Please specify your maximum capacity (maximum number of ambulance vehicles) and your availability to start. Please indicate the distance of your facility from the FNC located at 2300 Ramsey Street, Fayetteville, NC BUSINESS SIZE AND SOCIO-ECONOMIC STATUS: (a) Indicate whether your business is large or small (b) If small, indicate if your firm qualifies as a small, emerging business, or small disadvantaged business (c) If disadvantaged, specify under which disadvantaged group and if your firm is certified under Section 8(a) of the Small Business Act (d) Indicate if your firm is a certified Hub zone firm (e) Indicate if your firm is a woman-owned or operated business (f) Indicate if your firm is a certified Service-Disabled Veteran Owned Small Business (SDVOSB) or Veteran Owned Small Business (VOSB) (g) Include the DUNS number of your firm. (h) State whether your firm is registered with the System for Award Management (SAM) at www.sam.gov and/or the VetBiz Registry at www.vetbiz.gov. If not, please NOTE: any future solicitation could only be awarded to a contractor who is registered in SAM. To receive award based on VOSB or SDVOSB status you must be registered in the VetBiz Registry.
