Description
Please note this is NOT a request for quote or proposal. The Government is seeking information strictly for market research purposes only. The Government may or may not issue a solicitation document. The Richard L. Roudebush VA Medical Center (RLR VAMC), 1481 W. 10th Street, Indianapolis, Indiana 46202 is conducting a market survey for qualified firms capable of providing primary medical care and mental health services through a community-based outpatient clinic (CBOC) in the Terre Haute, Indiana geographical location. Applicable NAICS Code 621498 applies with a business size standard of $20.5 million and the contractor must be registered and verified in the Systems for Award Management System (SAM). Responses to this sources sought notice shall be submitted in writing (electronic is preferred method) and received no later than 4pm EST, Friday, December 30, 2016. No telephone inquiries shall be accepted or returned. A brief description of work to be performed includes the following: The RLR VAMC parent facility requires a contractor-owned, contractor-operated CBOC to provide primary care and mental health services, along with other specialty services, to approximately 5,000 Veterans living or residing in the Terre Haute, IN area for a 12-month base year with four option year renewals. Contractor shall be able to provide sufficient support staff to conduct daily business, including such functions as patient registration, financial assessments, and medical record documentation. The contractor shall provide personnel in numbers and qualifications capable of fulfilling the standards, as well as provide sufficient number of primary care providers so that each primary care provider has a caseload ratio to meet VA standards. The CBOC must follow the VHA PACT Model. Contractor shall be responsible for recruiting and retaining quality providers, as well as providing office space for other specialty services and clinics. The purpose of this sources sought notice is to gain knowledge of potential qualified sources and their size classification relative to NAICS Code 621498 (business size standard of $20.5 Million per annum). Responses to this sources sought notice will be used by the Government to make appropriate acquisition decisions. Reponses should include: company name, service capability statement, examples of similar facilities to which same or similar services were provided; DUNS number, including address, point of contact, and the size of the business pursuant to the following questions: (1) Is the business large or small? (2) IF small, does the firm qualify as small, emerging business or small disadvantaged business? (3) IF disadvantaged, specify under which disadvantaged group, and IF the firm is certified under Section 8(a) of the Small Business Act? (4) Is the firm a certified in VetPro? A solicitation is NOT currently available and may or may not be available in the future. IF a solicitation is issued, it will be announced on the Federal Business Opportunities (FBO) website at a later date, and all interested parties must respond to the solicitation announcement separately from the responses to this announcement.
