Sow Repair Olympus Urf-V2r December 15, 2016 VA Connecticut Healthcare System West Haven Campus Clinical Engineering Service SCOPE OF WORK Repair of Olympus URF-V2R 1. PROJECT DATA Repair of Olympus Ureteroscope Problem: Damage to distal tip and water invasion Location: Vendor s place of business- Olympus, 187 Billerica Road, Chelmsford, MA 01824 2. GENERAL DESCRIPTION OF THE WORK: The Contractor is an experienced and capable service repair provider and as such possesses comprehensive knowledge and ability to repair all three of the scopes for safe patient use and maximum diagnostic capabilities. The Contractor will evaluate the scope for damage and/or failure and report their findings to the VA. Once approved, the vendor will affect repairs according to the level of maintenance required to return the scope to full working condition. This may include up to a complete rebuild of the scope to the original manufacturer s specifications. The Contractor shall disassemble scope and reassemble with a new insertion tube unit, to include all internal insertion tube components: CCD with objective lenses, four angulation wires, light guide bundle with lenses, air/water channel, and biopsy channel. Replace/repair the following as needed: light guide connector, tension adjustment on control knobs, body control unit refurbishment, light guide tube, suction cylinder, air/water cylinder and tube replacement, light guide cover glass replacement and switch button replacement. Contractor shall leak test to ensure watertight integrity, check for proper insulation and restore to factory specifications as required.
