Description
This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested. Solicitation number VA263-16-Q-0406 is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-92. The VA anticipates one Fixed-Price Indefinite Delivery Indefinite Quantity Contract award for all resources and labor necessary to provide Advanced Life Support/Basic Life Support Ground Ambulance Transport Services for the VA Central Iowa Health Care System located at 3600 30th Street, Des Moines, IA 50310-5753. Additional requirements are listed within the attached solicitation document(s). A list of the contract line items, quantity, and unit of measure are listed within the attached solicitation document(s). This procurement is 100% set aside for Veteran Owned Small Business (VOSB). At the time of offer submission the offeror must represent it is a small business under the applicable NAICS. Prospective offerors must be registered and current in the System for Award Management (SAM) database prior to contract award: (https://www.sam.gov/portal/public/SAM/ ). Quotes will only be accepted from responsible contractors. The NAICS code for the solicitation is 621910 and the small business size standard is $15 million. The Defense Priorities and Allocation System (DPAS) does not apply to this solicitation. The provision at 52.212-1, Instructions to Offerors Commercial, applies to this acquisition. Please refer to the provision at 52.212-2 in the attached document for the specific evaluation criteria used for this solicitation. The provision at 52.212-4, Contract Terms and Conditions Commercial, and the provision at 52.212-5, Contract Terms and Conditions Required to Implement Statutes Or Executive Order Commercial Items also apply to this acquisition. Please refer to the attached document for a list of additional FAR clauses which apply to this acquisition. Areas to complete when submitting your quote: SF1449 completed and signed in blocks 17a, 30a, 30b, and 30c. Section B2 - Pricing information in the Price/Cost Schedule. A completed copy of the provision at 52.212-3. A separate narrative must also be submitted with your quote to address the technical and past performance evaluation criteria (See Section E, 52.212-2, Evaluation - Commercial Items). A site visit is not planned. If you would like to schedule a site visit please email your request to Michael.Manahl@va.gov. Site Visits must be scheduled and take place no later than 2:00 P.M. CST on January 5, 2017. All questions must be submitted in writing via email to Gregory.johnson12@va.gov. Questions must be received by January 9, 2017 by 2:00 P.M. CST. Quotes are due to the Contracting Office by Thursday, January 12, 2017 at 4:00 PM CST. Quotes are required to be emailed to Gregory.Johnson12@va.gov prior to the solicitation closing date and time. It is the responsibility of the offeror to ensure their quote is received in full by the Contracting Office before the solicitation closing. The Contractor, before contract award, must furnish the VA Contracting Officer with a Certificate of Insurance from his insurance company indicating that the coverage outlined in this solicitation has been obtained. After contract award and prior to commencement of work a pre-performance conference will be scheduled with the awardee.
