SOURCES SOUGHT NOTICE The Department of Veterans Affairs, Network Contracting Office (NCO) 15 is conducting market research to help determine the availability and technical capabilities of qualified vendors who can furnish all the Total Patient Nutrition (TPN) supplies for the Kansas City VA Medical Center. POTENTIAL SOURCES SHALL PROVIDE THE FOLLOWING INFORMATION IN THEIR RESPONSE: 1) Company name, address, phone number, primary contact(s), e-mail address, NAICS code(s), business size (i.e. small/large), and DUNS Number. 2) Statement of Capability that demonstrates ability of providing the item in accordance with the attached Statement of Requirement (SOR) and past performance in providing this type of service. Include examples of prior completed Government contracts, references, and other related information. This notice is to assist the NCO 15 in determining sources only. This announcement is not a request for proposals or quotations. The Government is not committed to award a contract pursuant to this announcement. The Government will not pay for any costs incurred in the preparation or submission of information in response to this announcement. Questions and responses must be submitted no later than 10:00 a.m. (CDT) on January 4, 2017 to Yvonne Kittling at yvonne.kittling@va.gov. Any proprietary information should be clearly identified as "proprietary information. Statement of Requirement (SOR) Kansas City VAMC Pharmacy requires a service contract to support the TPN manufacturing and compounding needs that are otherwise unavailable at from the Inpatient Pharmacy IV Room due to volume or sterile compounding requirements. TPN services are for patients currently admitted as Inpatient or otherwise under the care of the VA Medical Center. Scope This Vendor is to furnish all TPN supplies for the Kansas City VA Medical Center for a base year plus four option years. At this time, this contract is only for the Kansas City VA Medical Center, but currently the VA Heartland Network 15 consists of 8 hospitals and we may add other VA Medical Centers at a later date. All manufacturing and compounding will take place at the Vendors sterile manufacturing environment. The Vendor shall notify the Kansas City VAMC within 24 hours, in the event of any failed inspections, recalled items, or issues that have a direct impact on patient safety or the integrity of the manufactured items. Contractor shall provide 10% amino acids with 70% Dextrose in bags and will include additives such as vitamins, electrolytes, and Insulin. Contractor shall provide all necessary materials used in preparing the admixture solutions such as transfer sets, TPN clips, needles, syringes, seals, swabs, gloves and sharps containers. Mixing of TPN solutions shall be in compliance with the American Society of Hospital Pharmacies (ASHP), The Joint Commission (TJC) guidelines, Current USP 797/800 standards and any revisions or new USP standards identified over the course of the contract. Contractor shall label and number the TPN products with a label matching those currently used at the Kansas City VA Medical Centers. Usage records for each product admixed shall be provided by the Contractor on a monthly basis. Contractor shall provide quality assurance data as required by JCAHO on a quarterly basis. Contractor shall provide a customer service representative, which will be available 24 hours a day. Ordering: Ordering facilities will fax requests to the contractor. Figures will be adjusted according to usage and demand; numbers referenced above are only estimates and are subject to change. A method for electronic ordering is preferred over manually entered or faxing orders. Services to provide admixing, labeling, and delivery of the following Hyper alimentation Products: 10% Amino Acids 70% Dextrose in a bag plus minerals and electrolytes Insulin and heparin as requested 1001 to 2000 ml 2001 to 3000 ml Famotidine 20% lipids MVI
