1/3 The Department of Veterans Affairs Regional Office Indianapolis will post a written solicitation on 12/22/2016. This solicitation will be an SDVOSB set aside under NAICS code 561450. Below is the expected SOW, please see solicitation VA101V-17-Q-0020 for further details. Statement of Work Veterans Benefits Administration (VBA) Fiduciary Hub Credit Report Monitoring December 21, 2016 1. Background/Introduction: The VBA Fiduciary Hub maintains financial records for persons with disabilities who are incapable of maintaining financial accountability. The fiduciary Hub needs access to credit reports in order to determine if a particular person is suitable to be a fiduciary for a VA beneficiary. According to 38 U.S.C 5507 enacted in December 10, 2004, which provides authority for the VA to obtain credit reports from prospective fiduciaries. All presumptive fiduciaries will sign a letter allowing the VA to use their credit information. 2. Scope of Work: The contractor shall provide utilization to credit scores both electronically as well as in paper format. The contractor will allow access to credit information at all Eastern Area Fiduciary Hub locations to include: Baltimore Boston Providence New York Buffalo Harford Newark Philadelphia Pittsburgh Cleveland Detroit Manchester Togus White River Junction Wilmington Indianapolis The contractor will provide the following information as requested by the fiduciary hub at each location to include: Credit summary, including the following account information: Revolving (payments, balances, additional information), Installments (Payments, Balances, additional information), Real Estate (Payments, Balances, additional information) Credit Summary for Bankruptcies, Defaults, Judgments, and Past Due information Credit History Derogatory Summary Option for credit score Please quote with cost per individual line item usage, for example please see below Membership Fee- (one time charge) Recurring fees Credit reports/ cost per report FICO score (optional) Any Account minimums (monthly charges/yearly charges) 3. Estimated Usage: The Eastern Area Fiduciary Hub estimates an average need of 1,000 credit reports monthly or 12,000 annually. Place of Performance: Department of Veterans Affairs ATTN: Charles Brooks 575 N. Pennsylvania St. Room 362 Indianapolis, Indiana 46204 Period of Performance: Base Year: Effective date of award January 16, 2017. Plus four (4) option years.. Contract Administration of Task Order: The on-site Supervisor will serve as Government Project Officer (GPO). They will provide general instructions on limitations and deadlines, and is responsible for the administration of the task order in compliance with the contract. Changes and modifications can only be authorized by a warranted Contracting Officer. Billing: Services performed under this contract will be billed monthly. The contractor shall provide an invoice that will include: Contract Number Company Name and Address Invoice Date Invoice Number (Note: Each invoice must have a different Invoice Number) Description of items/services provided as well as referencing the appropriate Contract Line Item Number (CLIN). This way the proper CLIN can be billed for the Invoice. Terms for any discount for prompt payment Name, title and phone number of company point of contact to notify in case of a defective invoice 8. Type of Order: Firm Fixed Price/IDIQ
