Page 3 of 3 DIESEL FUEL 7,600 GALLONS; UNLEADED FUEL 1,400 GALLONS DELIVERED TO FORT SAM HOUSTON NATIONAL CEMETERY This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number VA786-17-0132 is being issued as a Request for Quote (RFQ). This procurement is 100% Small Business Set-Aside. The associated NAICS code is 324110 with a small business size standard 1,500 employees. NOTE: NAICS Code 324110 To qualify as small for purpose of Government procurement, the petroleum refiner, including its affiliates, must be a concern that has either no more than 1,500 employees or not more than 2000,000 barrels per calendar day total Operable Atmospheric Crude leased facilities, and all facilities under a processing agreement or an arrangement such as an exchange agreement or a throughput. To quality under the capacity size standard, the firm, together with its affiliates, must be primarily engaged in refining crude petroleum into refined petroleum products. A firm s primary industry is determined in accordance with 13 CFR 121.107. All responsible sources may submit a quotation, which shall be considered by the agency. Quotes may be submitted in hard copy or electronically, either through regular mail, email, or by facsimile. Regardless of method of delivery, it is the offeror s responsibility to ensure receipt of quotes by the time and date stated herein. Contractors shall submit a lump sum quotation on company letterhead to include all charges for the following items to be delivered in accordance with specifications below: CLIN 0001: Diesel Fuel 7,600 Gallons CLIN 0002: Unleaded Fuel 1,400 Gallons Delivered to the Fort Sam Houston National Cemetery, 1520 Harry Wurzbach Road, San Antonio TX 78209 DELIVERIES Deliveries Quick turnaround is necessary in order for the Government to fulfill its mission. Therefore, the fuel to be supplied under this purchase order shall be delivered to the Fort Sam Houston National Cemetery, 1520 Harry Wurzbach Road, San Antonio TX 78209. Prior to delivery, the Contractor shall coordinate delivery with the following personnel. Point-of-contact is Eric Von Bartheld (210) 820-2558 to schedule a site visit. The contractor shall provide both a unit price and total amount for each. Contractor shall submit completed clause 52.212-3, Offerors Representatives and Certifications, Commercial Items. The following clauses and provisions are incorporated and will remain in full force in any resultant award. LINE QTY UNIT TOTAL Diesel FUEL 7,600 GAL $________ Unleaded Fuel 1,400 GAL $________ $________ Performance Work Statement Concept: The contractor will provide bulk commercial diesel and gasoline delivered to the two fuel pods (500gal each) located at Fort Sam Houston National Cemetery. Contractor must be available for refuel operations between the hours of 8:00am- 4:00pm. Estimated fuel requirements will be given to the contractor no later than 12:00pm the day prior to delivery. Period of Performance: 1 December 2016 30 September 2017 Location: Refuel operations will take place at the Maintenance Yard located at Fort Sam Houston National Cemetery; 1520 Harry Wurzbach Rd San Antonio TX 78209. Scope: The contractor shall be responsible for providing bulk refuel operations on site as required. Contractor will ensure their equipment has the capability to couple with a D1 nozzle; contractor coupling will be a male 2 ½-3 inch cam lock to ensure seal and transfer connection. Safety and Environmental: The contractor shall be responsible for furnishing all materials, labor, and equipment necessary to meet safety regulations related to refueling operations of this nature. Customer will coordinate with host site for approval to conduct refuel operations. Customer will provide spill containment during refuel operations at each location. Contractor will BPT provide spill clean-up support in the event of an incident during refuel operations. Dates Locations Estimated Fuel: Location is non-changing and will be confirmed upon award of contract with final confirmation not less than 72 hours prior to delivery. Fuel quantity is estimated and will be confirmed prior to delivery for reduction in requirement based on ongoing refined estimates.
