Sources Sought The Department of Veterans Affairs, VISN 22, Network Contracting Office, intends to release a solicitation under FAR Part 13 (Simplified Acquisition Procedures) to request supplies from a contractor that can supply CPAP materials at 11301 Wilshire Blvd, bldg. 500, 3N, Room 3025, Los Angeles, CA. 900712. REQUIRED SUPPLIES: The required equipment is as follows: Model Quantity Description DSX500H11 50 Dreamstation auto CPAP with Humidifier 100955 20 SleepWeaver Anew Mask & headgear size small 100960 50 Sleepweaver Anew Mask & headgrear size regular 100965 40 Sleepweaver Anew Mask & Headgear size Large *** Exact match only no substitutions. Interested parties must submit an unclassified Statement of Capability (SOC) demonstrating capability of services required and whether they can provide these services under open market. The SOC must include the following: business name, point of contact name, telephone, email address, DUNS number, GSA contract if applicable, business size (small, large) and small business designation (small, veteran own, service disable, women owned, etc.). This notice does not constitute a Request for Quote (RFQ). Information herein is based on the best information available at the time of publication, is subject to revision, and is not binding on the Government. The Government will not recognize any cost associated with the submission of a SOC. Submit all responses not later than 8:00 a.m. Pacific Standard Time on Tuesday December 20, 2016 to the attention of Robert Regan at Robert.regan2@va.gov with Interested Contractor CPAP Dreamstation contract on the subject line.
