- Performance Work Statement Solicitation Amendment 01 Solicitation DESCRIPTION OF SERVICES: The contractor shall provide the non-personal services of Protestant Musician at Tripler Army Medical Center, Hawaii. The musician is to provide appropriate musical support during the designated worship service. Musician must be intimately familiar with the Protestant Services and its liturgical requirements to provide musical leadership for the Sunday Protestant Services. Lead and select the proper hymns, choruses, and spiritual songs; perform preludes; postlude; offertory; and background for meditation and prayers; that enhances the atmosphere of worship for the particular congregation. Play prelude 15 minutes prior to designated Worship Service/Ceremony start time and postlude of at least 5 minutes at the conclusion of the Worship Service/Ceremony. Location. Tripler Army Medical Center, D-Wing, 3rd Floor room 3D-165. Hours. The following current schedule of events is provided for planning purposes, and is subject to change by publication in the weekly chapel bulletin and/or announcements during the Worship Service. Thursday: Rehearsal for Sunday Services - 1700-1900 Sunday: Protestant Services - 0845-1005 (Also, Special Occasions, i.e., Easter, Christmas, Memorial Services, etc.) SOLICITATION This solicitation will be 100% set aside to small business using lowest price technically acceptable source selection process when best value is expected to result from selection of the technically acceptable quote with the lowest evaluated price. Award will be firm fixed priced (FFP), commerical services type contract, based on FAR 12 Acquisition of Commerical items and FAR Subpart 13.5 Simplified Procedures for Commerical Items. PERIOD OF PERFORMANCE The period of performance is anticipated to be one 12 month base period and two 12 month option periods utilizing FAR 52.217-8-Option to extend services for six (6) additional months for a total duration of 42 months. Base: 1 Jan 2017 - 31 Dec 2017 1st Option 1 Jan 2018 - 31 Dec 2018 2nd Option 1 Jan 2019 - 31 Dec 2019 PRICING Offeror's price quote shall include completed pages 3 through 5.
