- THIS IS A SOURCES SOUGHT NOTICE REPRESENTING A MARKET SURVEY AND IS NOT A REQUEST FOR PROPOSALS, PROPOSAL ABSTRACTS, QUOTATIONS OR INVITATION FOR BIDS The Department of Veterans Affairs is conducting a MARKET RESEARCH SURVEY to obtain information regarding: (1) the availability and capability of qualified large business sources (2) the availability and capability of qualified small business sources; (3) whether they are Service-Disabled Veteran-Owned, Veteran-Owned, Small Business, HUB Zone 8(a), Women-Owned, or small disadvantaged business concern, (4) their size classification relative to the North American Industry Classification System (NAICS) code for the proposed acquisition, and (6) current market price. Your responses to the information requested will assist the Government in determining the appropriate acquisition method, including whether a set-aside is possible. The NAICS code is 339113 DESCRIPTION OF THE REQUIREMENT: VARIOUS STRYKER ORTHOPEDIC SUPPLIES FOR CONSIGNMENT AGREEMENT The Albany VA Medical Centers has a requirement for various Stryker orthopedic parts and instruments to be supplied to the facility under consignment agreement terms. The vendor would be responsible for maintaining and stocking the inventory, without charge to the VA. Payment would be issued for items used from the inventory only. Interested vendors must be authorized Stryker distributors. No gray-market items will be allowed within the facility. Projected inventory to be made available onsite at the Albany VA Medical Center includes: Variax Clavicle Set Gamma Disposable Instrument Set Various plates Superior place decreased (various side/holes) Anterior midshaft plates (various side/holes) Plate inserts Forceps Various screw, locking screws Variax clav tray (1 and 2 level) Tube clips Guide wire Drill bits *A complete list of required parts will be provided during the solicitation period. This list is only to be used for market research purposes to assess interest and capability. Albany VA Medical Center: 113 Holland Ave. Albany, NY 12208 POTENTIAL SOURCES SHALL PROVIDE THE FOLLOWING INFORMATION IN THEIR RESPONSE: 1) Company name, address, phone number, primary contact(s), e-mail address, NAICS code(s), business size (i.e., small/large, VOSB, SDVOSB), FSS/NAC/GSA contract number (if applicable), DUNS Number and approximate pricing for VARIAX CLAVICLE SET. Pricing will be used for market research and planning purposes only. Sample pricing for any other parts reflective of the list above is also welcome. 2) Responses may be emailed to delene.kendrick@va.gov. 3) The deadline for responses to this notice is 4:00pm. EDT on Thursday December 15, 2016. AT THIS TIME NO SOLICITATION EXISTS. (DO NOT REQUEST A COPY OF THE SOLICITATION). AN AWARD WILL NOT BE MADE AS A RESULT OF THIS RFI.
