Page 2 of 2 This is a SYNOPSIS / PRE-SOLICITATION NOTICE for a commercial requirement for Outpatient Telemetry and Event Monitoring at the Hunter Holmes McGuire VA Medical Center (VAMC), Richmond, VA. The North American Classification System (NAICS) Code for this requirement is 621512 with a size standard of $15.0 M. This acquisition is a 100% Small Business set-aside. A Firm Fixed-Price type contract with a base period and up to four (4) one (1) year option periods, to be exercised at the Government's discretion is contemplated under this solicitation. A formal Request for Proposal (RFP) in accordance with Federal Acquisition Regulation (FAR) Part 12 Acquisition of Commercial Items and FAR Part 15 Contracting by Negotiation will be issued by December 7, 2016. Sources capable of meeting the requirements of the PWS are encouraged to provide their proposal to provide all labor, material, tools, equipment, maintenance and repair of all Contractor furnished equipment, specialized staff, training, required supplies to perform Outpatient Telemetry and Event Monitor Services at Hunter Holmes McGuire VA Medical Center (VAMC). Contractor furnished equipment shall include but not be limited to a Mobile Cardiac Outpatient Telemetry and Event Monitoring equipment with staff monitoring and reporting to the VA. Monitoring devices shall be the most current level of knowledge and technology and have an algorithm program to recognize rhythms such as Ventricular Fibulation (VFib), tachycardia, bradycardia, and systole. Equipment Features and Minimal Requirements are as follows: 1 or 2 Channel 2 or 3 lead-wires Event recordings up to 30 days Transmission over cell phone network Continuous monitoring & recording Patient activation and Auto-trigger activation A-fib, Brady, Tachycardia, and Pause detection Storage on device for analysis and review Asymptomatic patient, Periodic symptoms. Need for auto trigger feature. Ability to view all the data if necessary. Contractor monitoring services shall include as a minimum: Function 24 hours per day, 7 days per week and 365 days a year. Provide 24 hour Mobile Cardiac Outpatient Telemetry and Event Monitoring from a 14 to 28 day period. Provide detail assessment of 24-hour ventricular ectopy count and burden during the 14 to 28-day period. Be staffed with Certified Cardio Graphic Technicians, Emergency Medical Technician (EMT) with Advanced Cardiovascular Life Support (ACLS), or Registered Nurse (RN) with ACLS. Receive and process both patient triggered and asymptomatic Electrocardiography (ECG) events and notifies physician immediately when an urgent event meeting the physician notification criteria is received. Provide the following reports: Daily Report sent to physician for the previous 24 hours and includes representative ECGs from the prior 24 hours, any ECGs meeting the physician notification criteria, a heart rate trend graph and if applicable an AF burden graph. Urgent Report prepared when an ECG meets the physician notification criteria. End of Service Summary Report within 3-5 days after the monitor is shipped back to contractor. The Contractor shall meet all Federal, State and Local codes and requirements for the operation of this service and equipment provided. Contractor personnel performing services shall be licensed and/or registered in a State, Territory or Commonwealth of the United States or the District of Columbia. Potential Sources must be registered in SAM.
