- Contract Clauses and Provisions Item List This is a combined synopsis/solicitation for commercial services prepared in accordance with the format prescribed in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Solicitation FA4877-17-Q-0077 is issued as a request for quotation (RFQ). This solicitation document incorporates provisions and clauses of those in effect through Federal Acquisition Circular 2005-92, Defense Circular Number 20161104, and Air Force Acquisition Circular 2016-0603. This procurement is being issued as a 100% small business set-aside. The North American Industry Classification System Code is 315990 with a size standard of 500 employees or less. The right to make multiple or no award is reserved in the event it is advantageous to the Government to do so. Please see attachment 1, Item List. The contractor shall provide all items on said list. Please price out your quote breakout with, unit, and total pricing for each item on the list. All items are requested as "Brand name or Equal". Contractors shall include part number and manufacturer for each substituted item. Any "Brand Name Or Equal" sunglasses must be APEL approved, include interchangeable lenses, provide ballistic fragmentation protection, be similar in style, and meet all ANSI Z87.1-2010 optical and safety requirements. Delivery: Delivery shall be FOB Destination, and MUST be completed by 30 Dec 16. Please include any/all transportation costs in the line items. Please price out your quote breakout with pricing for each item on the list. Contractors may include expedited shipping costs in their quote if necessary to meet the delivery date of 30 December 2016. SEE ATTACHMENT 2, CONTRACT CLAUSES AND PROVISIONS, FOR ALL APPLICABLE CONTRACT CLAUSES AND PROVISIONS. QUOTES WILL BE EVALUATED ON TECHNICAL ACCEPTABILITY AND LOWEST PRICE Any questions pertaining to this acquisition shall be submitted in writing no later than Wednesday, 7 December 2016 by 2:00 PM Arizona Local Time. Written questions shall be sent to SSgt Branden Reagan at branden.reagan@us.af.mil. NO questions will be considered after close of questions due date. It is the responsibility of the offeror to view the website for any changes or amendments to this RFQ. Quotes will be due to the 355th Contracting Squadron, 3180 S. Craycroft Rd. Davis-Monthan AFB, AZ 85707, via email Thursday, 8 December 2016, at 4:00 P.M, Arizona Local Time. It is the responsibility of the vendor to ensure the quote was received by 355 CONS on time.
