- Attachment 4 Past Performance Questionnaire Attachment 3 Pricing Worksheet Attachment 2 Requirements Attachment 1 Request for Proposal This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is #HSCG23-17-R-MOT001 and is request for proposal (RFP). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-92. The U.S. Coast Guard Acquisition Directorate anticipates awarding a Firm Fixed-Price Requirements Contract with (1) base and (2) option period to procure up to Ten (10) Special Purpose Craft-Ice Rescue Transport (SPC-ICT's) in accordance to the Requirements - Attachment 1. This combined synopsis/solicitation is being posted in accordance with FAR 13.5 Test Program for Certain Commercial Items and evaluated in conjunction with FAR 15.101-1. The contract will be issued to one (1) Offeror capable of successfully supplying the SPC-IRT, found to be the best value to the Government in accordance with FAR part 15.101-1. Contractors shall review (Attachment 1) for supplemental solicitation information described in FAR Part 12.301. The contractor shall perform all functions necessary to provide the commercial item described herein. Each deliverable shall be provided as per the scope of CLINS and in accordance with the Requirements document referred to as (Attachment 2) and will be incorporated into the contract upon award. Manufacturing shall comply with all applicable Federal, State, and local laws, regulations, codes and directives. Contractors shall use the Pricing Spreadsheet (Attachment 3) and the Past Performance Questionnaire Rating Sheet (Attachment 4) in the development of the response to the solicitation. This solicitation is being issued under NAICS Code 336612 (size standard: 500 or fewer employees) as unrestricted.
