Description
THIS IS A SOURCES SOUGHT NOTICE. This is not a request for proposals. A solicitation will not be issued at this time. The Department of Veterans Affairs, Northern Arizona VA Health Care System (NAVAHCS) has a requirement for Dimentia Patient Dining Chairs. The intent of the procurement is to award a firm-fixed price contract. The Statement of Work (SOW) defines the effort required for the purchase and delivery of the system to the Prescott VA. Unique Features: NAVAHCS Chairs required must meet or exceed the following specifications: 1.Purpose: Northern VA Health Care System (NAVAHCS) has the requirement for 20 chairs for the Dementia Unit of the Community Living Center (CLC). The current dining room chairs have been deemed unsafe for nursing home patients and were the subject of an internal RCA due to a patient injury and subsequent death. The chairs required must have weighted bottoms and with perfectly straight leg construction in a risk-adverse measure to minimize falls should an elderly patient lean on them for any reason. For this reason best value to the government will be weighted highest in the evaluation versus LPTA. 2.Requirements - Chair MUST meet the following: a.Chair Construction (1)Arm panels (open arm), six-quarter (6/4) domestic beech hardwood, kiln dried to moisture content of 6% to 8% at time of arm construction. (2)Assembly joinery utilizes both glued finger-joint and double-dowel styles. (3)Arm Caps - black poly; attached by 3 metal dowel/screws per arm pad b.Back Construction (1)One (1) part compound-curve laminated plywood. (2)Outerback shell is ½" 6-ply and is attached to arm using a structural steel angle 1-1/8" x 1-1/4" x 1/8" on open arm and structural grade 2 hardware c.Seat Frame (1)Fully welded structural steel angle frame (1" x 1" x 3/16"). (2)Frame is bolted to wood arms using structural grade fasteners. d.Glides must be nylon e.Foam Specifications (1)Seat must be contour-cut, Ultra cell (High Resilience) polyurethane foam. 3" in thickness with specifications of 42 lbs. compression and 2.5 density. (Front of seat edge is composed of 55 lbs. compression). (2)Back must be Ultra cell (High Resilience) polyurethane foam. 2" in thickness with 32 lbs. compression and 2.5 density, molded wood veneer arm inserts are cut to size. Attachment to arm is made using dowels that insert into arm. f.BIFMA Testing (1)Must meet all applicable requirements of the ANSI/BIFMA X5.4-2012 Lounge Seating test standard. g.Weight rating must be 300 lbs. h.Armrest Durability (1)Must pass Horizontal Cyclic 100 lb. horizontal force applied to the armrest 50,000 times. (2)Must pass Vertical Cyclic 150 lb. vertical force applied to the armrest 10,000 times. i.Armrest Strength (1)Must pass Horizontal Functional Load: 133 lb. horizontal force applied to the armrest one time. (2)Must pass Proof Load: 200 lbs. horizontal force applied to the armrest one time. (3)Must pass Vertical Functional Load: 200 lbs. vertical force applied to the armrest one time. (4)Must pass Proof Load: 300 lbs. vertical force applied to the armrest one time. Sellers must be able to document their ability to provide items through manufacturer approved distribution channels upon request. The Seller confirms to have sourced all products submitted in this Bid from manufacturer-approved channels for Federal sales, in accordance with all applicable laws and manufacturers current applicable policies at the time of purchase. Seller must be able to support both the product sale and product pricing, in accordance with applicable manufacturer certification/specialization requirements. If software is provided or included, Seller shall, upon request, provide Buyer with a copy of the End User License Agreement. Seller certifies that all software is licensed originally to Buyer as the original licensee authorized to use the software. 5.Delivery Charges: All delivery charges must be included in the price of the items. 6.Technical Support: None required SOUTHERN ARIZONA VA HEALTH CARE SYSTEM (SAVAHCS): VAMC: 3601 S 6th Ave, Tucson, AZ 85723 The applicable NAICS code is 337127 Institutional Furniture Manufacturing. Small Business manufacturers for this NAICS code have a size standard of 1,000 employees. The resulting contract will be a firm fixed price order. The term of the contract is intended to be 60 calendar days ARO, with no options included. It is anticipated that a Request for Quotes will be issued in September 2016. Award of a firm fixed price contract is contemplated by September, 2016. This notice is published to conduct market research to determine if there is a sufficient number of certified Service Disabled Veteran Owned Small Businesses (SDVOSB), Veteran Owned Small Businesses (VOSB), Small Businesses (SB), or Large Business capable of performing the requirements to warrant a socio-economic set-aside or full & open competition. It is strongly recommended that interested parties register in the US Department of Veterans Affairs Vet Biz database and the Small Business Administration (SBA) database. The response date to this Sources Sought notice is September 17. 2016 at 1200pm EST. This market research is for informational and planning purposes only to determine if a socio-economic set-aside, or full & open competition, or sole-source procurement is appropriate.
