Description
The Department of Veterans Affairs Medical Center, VISN 6 Network Contracting Office (NCO), on behalf of Durham VA Medical Center, is conducting market research to determine small business, small disadvantaged business, 8(a), Service Disabled Veteran Owned Small Business (SDVOSB), Woman Owned Small Business or HUBZone's interest for Blood Bank Reagents, per attached Statement of Work. The North American Industry Classification System (NAICS) Code for this acquisition is 325413, Size Standard: 1.250 employees, and the Classification code is 6550. THIS IS A SOURCES SOUGHT NOTICE ONLY. This notice does NOT constitute a request for proposal, request for quote, or invitation for bid. No formal solicitation document exists at this time. The Government will not pay for any information solicited. Interested parties should submit with their response the following information: 1.Company name and DUN # 2.POC information 3.Business size and socio-economic classification (for example SDVOSB, Woman-owned) 4.Is this item available on FSS Schedule? If so, provide contract #. Please email this information to john.summers2@va.gov no later than September 20, 2016. Phone inquiries will not be accepted. CLINICAL BLOOD BANK REAGENTS STATEMENT OF WORK SCOPE: The Durham VA Medical Center will need red cell reagents to perform ABO/Rh, antibody screening direct antiglobulin testing, and antibody identification as part of compatibility testing for our patient population. These reagents must be provided to the Blood Bank Section VAMC Durham, 508 Fulton Street, Durham, NC per the estimate given in the Statement of Work. GENERAL REQUIREMENTS 1.1.The vendor will furnish all clinical laboratory supplies as described herein to VAMC Durham in accordance with the applicable Federal Supply Schedule contract on or before the beginning of the period of performance. This contract will be fixed-price for the duration of the contract in accordance with the terms and conditions of the contractors' Federal Supply Schedule. 1.2.The reagents will meet the sample volume in Attachment 1. 1.3.The vendor will provide each participating facility with Food and Drug Administration (FDA) approved reagents on or before the contract effective date; unless an alternate date is agreed upon by the vendor and the Government. Vendor must list the consumable parts provided. Vendor will state any parameter/function that is pending FDA approval at the time of this contract submission. 1.4.The proposed reagents must have at least one year proven experience in medical laboratories and be available on the Federal Supply Schedule. SUPPLY REQUIREMENTS: 1.5.The reagents shall be able to perform ABORh Blood Type, antibody screening using 3 reagent red blood cells, antibody identification, direct antiglobulin testing, and antiglobulin confirmation (Coombs control) testing 1.6.The vendor will provide VAMC Durham, for the duration of the contract, with all reagents, and controls, the vendor must include enough supplies (listed above) to perform repeat testing for 10% of the patient samples. All materials must meet CAP and AABB requirements. 1.7.The vendor shall list all reagents needed for blood bank testing, state if the reagents are stored at room temperature or refrigerated, and state the format/size of the reagents. The vendor may state if reagents, controls, or calibrators are color-coded.
