- FA3016-16-R-0049 Amendment 1 AMENDMENT 3 15 September 2016 Summary of Changes: - Adding the SF30 Amendment document - Answer Question from the Customer Question 1: Will there be SF 30 forms issued for the two amendments that are to be included in the proposal or should the amendment acknowledgement (shown in Volume I, item 5 of the proposal; page 34 of the solicitation) just be noted on page 2 of the SF 1442? Answer: Please fill out the attached SF30 and send it in with the proposal. Also make sure to acknowledge the amendment on the SF1442.
