- ON September 15, 2016 at 4:50PM EST and In accordance with FAR 19.502-2(a) the small business set-aside is withdrawn. This requirement is being resolicited on an unrestricted basis. Physical Exam This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The North American Industry Classification System Code (NAICS) is 621111 Offices of Physicians (except Mental Health Specialists) with a corresponding small business size standard not to exceed $11,000,000 dollars. This acquisition is 100% set aside for small business concerns. This is a Fiscal Year 17 requirement. There are currently no funds available. This requirement is subject to the availability of funds. The 439th Airlift Wing requires pre-employment and annual physical examinations for Civilian Firefighter/Security Forces/Emergency Management/ Bio-Engineering. We are looking for a Physician- a licensed doctor of medicine (M.D.) or osteopathy (D.O.) who has been designated by the agency to provide professional expertise in the areas of occupational safety and health as they relate to emergency services. See attached Performance Work Statement. Candidates will go to doctors office for pre-employment physical. All quotes will be considered. Any resultant award will be made to the lowest priced offeror providing a technically acceptable offer. The Government anticipates a firm fixed price contract. The following Federal Acquisition Regulations (FAR) and its supplements (DFARS and AFFARS) provisions and clauses apply to this acquisition; 52.232-33 SAM Registration (Payment by Electronic Funds Transfer - System for Award Management) 252.232-7006 (WAWF), (Wide Area Workflow Payment Instructions) 252.232-7003, Electronic Submission of Payment Requests and Receiving Reports 252.204-7004 Alternate A, System for Award Management All prospective contractors must be registered in the System for Award Management (SAM) to be eligible for award.
