- 1. PURCHASE REQUEST: 600-16-4-087-7334, 600-16-4-087-3538 VA262-16-AP-8925, VA262-16-AP-91931A. PROJECT/TASK NUMBER: NA 1B. ESTIMATED AMOUNT: $248,091.00 2. BRIEF DESCRIPTION OF SUPPLIES OR SERVICES REQUIRED AND THE INTENDED USE: STOCK Concentrators, Ventilators, and associated accessories (EverFlow and Millennium Concentrators and Trilogy 100 Ventilators, as well as associated accessories). These supplies are medical devices most often used in the home to provide a continuous oxygen source or assistance in breathing and a better quality of life for patients. When Clinicians prescribe these items they are usually needed promptly. Therefore, having these STOCK supplies on hand will make it possible for the items to be readily provided to patients. 3. UNIQUE CHARACTERISTICS THAT LIMIT AVAILABILITY TO ONLY ONE SOURCE, WITH THE REASON NO OTHER SUPPLIES OR SERVICES CAN BE USED: First Nation Group, LLC/Jordan Reses Supply Company, LLC is the exclusive supplier of Phillips Respironics products to the VHA, so no RFQs were sought. In the past, RFQs have been requested from other FSS, VA NAC contract holders who carry similar items. No Vendors were able to provide Quotes and they advised that it is because they are not authorized distributors of Phillips Respironics products. 4. REASON THAT SUGGESTED SOURCE IS THE ONLY SOURCE, WHICH CAN PROVIDE THE SUPPLIES OR SERVICES: All the requested items are Philips Respironics products and since First Nation Group, LLC/Jordan Reses Supply Company, LLC is the exclusive supplier of these products to the VHA, then any such products received from a Vendor other than First Nation Group, LLC/Jordan Reses Supply Company, LLC may result in charges to the VA for warranty work. Therefore, this is a Sole Source acquisition with the monetary majority of the required items on First Nation Group, LLC/Jordan Reses Supply Company, LLC's FSS, VA NAC contract V797P-4297b. 5. DESCRIPTION OF MARKET RESEARCH CONDUCTED AND RESULTS OR STATEMENT WHY IT WAS NOT CONDUCTED: Market Research was conducted as noted above and it was also determined that First Nation Group, LLC/Jordan Reses Supply Company, LLC are registered in SAM until 08/04/2017 and 10/20/2016 respectively with no Exclusions found. First Nation Group, LLC/Jordan Reses Supply Company, LLC are in the process of name/contract novation. 6. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106-1(b).
