- Attachment 3 – Specifications Attachment 2 – Clauses & Provisions Attachment 1 – Line Item List Combined Synopsis Solicitation -Fabrication of Test Fixtures Response Date: 22 September 2016 at 12:00 (Noon) PM Mountain Standard Time Solicitation No: W9124R-16-T-0058 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, streamlined solicitation for commercial items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Request for Quotes (RFQ) are being requested and a written solicitation will not be issued. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-89, Effective 15 Aug 2016 and Defense Federal Regulation Supplement (DFARS), current to DPN 20160830 (Effective 30 August 2016) Edition. Offerors that fail to furnish required representation or technical information as required by Federal Acquisition Regulation provisions 52.212-1 or reject the terms and conditions of this solicitation shall be excluded from consideration. It is the responsibility of the contractor to be familiar with the applicable provisions and clauses which can be accessed in full text at www.farsite.hill.af.mil. This solicitation is being issued as a Request for Quotations (RFQ) under Solicitation Number W9124R-16-T-0058 which will result in a single-award, firm fixed price contract. This action is being solicited as a 100% small business set aside. The North American Industry Classification System (NAICS) code is 332999 - "All Other Miscellaneous Fabricated Metal Product Manufacturing," with a size standard of 750 employees. Quotes will be evaluated on a Lowest Price Technically Acceptable (LPTA) basis and tradeoffs are not permitted. An offeror must quote on all items in this solicitation to be eligible for award. These items will be acquired using FAR Part 13 procedures. The United States Army Yuma Proving Ground (USAYPG), Yuma, Arizona is requesting quotes for a two year single Indefinite Delivery, Indefinite Quantity (IDIQ) award for Fabrication of Test fixtures, reference Attachment - 1. The Government's obligation is limited to the minimum guarantee of two fixtures and a maximum of 11 fixtures. The initial order will be provided at the time of award and will specify the types and quantities of fixtures that are to be delivered. Total quantities ordered (except for the initial quantity) during the award period may be more or less than the estimated quantity for the individual Fixtures listed. However, the total quantities ordered will not exceed the maximum amount for the ordering period. This contract will consist of Contract Line Item Number (CLIN) 0001, Quantity: 1, Unit of Issue: Each, Description: A Adapter Plate, 1.5 in. thick, 48 inch square, 4 in. x 4 in. hole pattern to 8.75 in. x 8.75 in. pattern with thermal barrier; CLIN 0002, Quantity: 1, Unit of Issue: Each, Description: B Cargo Plate, 1.5 in. thick, 48 inch square, 4 in. x 4 in. hole pattern; CLIN 0003, Quantity: 3, Unit of Issue: Each, Description: C Cargo Plate, 2 in. thick, 54 inch square, 4 in. x 4 in. hole pattern; CLIN 0004, Quantity: 6, Unit of Issue: Each, Description: D Cargo Plate, 1.5 in. thick, 60 inch square, 4 in. x 4 in. hole pattern; CLIN 0005, Quantity: 1, Unit of Issue: Each, Description: E Spacer Plate, 1 in. thick 48 inch square; CLIN 0006, Quantity: 1 Unit of Issue: Each, Description: F UD T2000 Armature head plate, 2 in. thick, 2 in. x 2 in. hole pattern and CLIN 0007, Quantity: 2, Unit of Issue: Each, Description: G UD T2000 Armature L fixture, 2 in. x 2 in. hole pattern. All quotes shall include transportation costs to USAYPG Yuma, Arizona 85365-9498 and be clearly marked with solicitation number W9124R-16-T-0058 and emailed to the Point of Contact (POC) listed below or sent by facsimile to 928-328-6534. Arizona vendors are to include Arizona Transaction Privilege Tax. Shipping and taxes (if applicable) shall be incorporated into your proposed price. Payment will be made through Wide Area Work Flow (WAWF). Delivery shall be made to USAYPG, AZ 85365-9498 with FOB Destination. All responding vendors must be actively registered with System for Award Management (SAM) prior to award of contract. Information to register in SAM can be found at https://www.sam.gov/portal/public/SAM. This CSS closes on 22 September 2016 at 12:00 (Noon) PM Pacific.
