- AID1420-17 Contractor Employee Biographical Data Sheet Contractor Performance Report - Short Form Past Performance Information Form VAT Reimbursement Processes Solicitation The United States Government, represented by the United States Agency for International Development Mission in Ethiopia (USAID/Ethiopia) through the Offices of Acquisition and Assistance (OAA) and Health, AIDS, Population and Nutrition (HAPN), is seeking proposals from qualified organizations interested in providing the services as described in this solicitation. This procurement is going to be conducted through full and open competition for which the procedures set forth in FAR Part 15 will apply. The geographic code for this Solicitation is 935, Primary Product/Service Code is R410, NAICS code is 541990, and the procurement instrument is firm fixed-price type contract. Subject to the availability of funds, USAID/Ethiopia intends to procure a new Firm Fixed Price (FFP)contract to perform an end-line performance evaluation of the Yekokeb Berhan Highly Vulnerable Children (HVC) activity. The purpose of this evaluation is to analyze the effectiveness of the activities' systems strengthening approach to improve access to quality services and health outcomes for HVC and their families and to inform the design of a follow-on activity.
