This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is RFP160025 and is being issued as a Request For Proposals (RFP). The associated NAICS code is 621111. See Schedule B of the attached RFP package for the list of contract line item number(s) and items, quantities and units of measure. The Office of the Architect of the Capitol (AOC) requires physical examinations for all workers who may be occupationally exposed to hazardous materials or conditions. The objective of this procurement is to establish a single-award time and materials contract for the medical evaluations of AOC employees. The AOC seeks to assure the health and safety of all employees and to comply with all relevant Health and Safety regulations that require medical evaluations. The period of performance is as follows: Base Year: November 1, 2016 - October 31, 2017 Option Year 1: November 1, 2017 - October 31, 2018 Option Year 2: November 1, 2018 - October 31, 2019 Option Year 3: November 1, 2019 - October 31, 2020 Option Year 4: November 1, 2020 - October 31, 2021 Place of Performance: It is expected that all physical examinations will be conducted in the OAP Health Unit located at B-344 Rayburn House Office Building. In the event that facility should be unavailable or in certain exigent situations, the examination will be conducted at the clinical location of the contractor. The Offeror shall have a fixed location no more than 25 miles from the U.S. Capitol Building. See the Statement of Work (SOW) for the full listing of required services and terms and conditions included in the RFP package for all applicable terms and conditions to this procurement. Defense Priorities and Allocations System (DPAS) is not applicable to this procurement. Proposals are due to the Contracting Officer, via email at jhenley@aoc.gov, and the Postal Operations Screening Facility no later than September 16, 2016 at 5:00 pm ET. With each copy of the form entitled "SOLICITATION, OFFER, AND AWARD", the offeror shall enclose the completed Schedule page; offer guarantee, if required; Representations and Certifications, and SF 3881 ACH VENDOR/MISCELLANEOUS PAYMENT ENROLLMENT FORM.
