Request for Proposal Solicitation #: VA243-16-R-0699 ISSUE DATE: 08/19/2016 PROPOSAL DUE DATE: 09/02/2016 AT 2:00 PM, EDT Project Number: 632-16-3-6063-0164 Project Title: OR Flooring Replacement Location: Northport VAMC, 79 Middleville Rd, Northport, NY 11768 MAGNITUDE OF CONSTRUCTION: Between $25,000 and $100,000 NAICS 236220 Small Business Size Standard is $33.5 Million PROJECT SUMMARY: Provide all labor, materials, equipment, transportation, supervision and disposal necessary for project 632-16-3-6063-0164, OR Flooring at Northport VAMC, Northport, NY 11768 All work in accordance with this Scope of Work, Drawings, Wage Determination, and technical requirements per all Federal, state & local codes. All references to "Bid" or "Bidder" shall be understood to mean "Offer" or "Offeror". SITE VISIT: An organized site visit is scheduled at 1 PM, 08/26/2016. Meet in Bldg. 6 EMS Office - 1st Floor Room 106 at the Northport VAMC. Award is subject to availability of funds REQUEST FOR INFORMATION: Submit all requests for information from the prime contractor to the CO via email. The deadline to submit electronic Requests for Information by the prime contractor is by 1:00PM EST on August 30, 2016. SUBMISSION OF PROPOSALS: All proposals must be submitted via email. OFFER DUE DATE: September 2, 2016 AT 2:00PM EDT To be considered and accepted as timely the proposal must be uploaded in its entirety prior to the deadline. Failure to provide a proposal in its entirety prior to the deadline will render the proposal noncompliant. If it is received in part it will not be considered timely, unless the offeror can show the proposal was uploaded by 5:00 pm one working day prior to the deadline per FAR 15.208 (b)(1)(i). ______________________________________________________________________________: BASIS FOR AWARD: Award will be based on lowest price. Offerors must submit a cost breakdown in sufficient detail to permit a complete analysis of labor burden, materials, equipment, transportation, supervision and disposal. When price is the only factor considered, award will be based on the lowest reasonable price received. Proposals unreasonably high or unrealistically low in price, when compared to the Government estimate and market conditions evidenced by other competitive proposals received, may be indicative of an inherent lack of understanding of the solicitation requirements and may result in proposal rejection without discussion. It is intended that award will be made on the Base Offer. Copies of all major subcontractor quotes are required. Placement procedures for all task orders include consideration of the following: Contractor Responsibility; Past performance on earlier orders under the contract, including quality, timeliness, and cost control; Potential impact to ongoing performance of other orders placed with the contractor; and Minimum order requirements. Cost Proposal: Cost will be evaluated as Lowest price. GENERAL INFORMATION: This solicitation is for a firm, fixed price construction contract. Award shall be made to the offer that provides the lowest price technically acceptable. The Government intends to award a contract to one offeror without discussions. NOTE: General Wage Decision Number NY160012 08/12/2016 NY12 is hereby incorporated into this solicitation, however it is the responsibility of the contractor prior to the due date of the proposal to regularly check the wage decision so as to include the most current prevailing wage in their proposal. SUBMISSION OF PROPOSAL: Offerors are cautioned to read the requirements of VAAR 852.219-10, VA NOTICE OF TOTAL SERVICE DISABLED VETERAN OWNED SMALL BUSINESS SET-ASIDE (DEC 2009); paragraph (c) (4), Construction by general contractor, at least 15 percent of the cost of the contract performance incurred for personnel will be spent on the concern's employees or the employees of other eligible service disabled veteran-owned small business concerns. The offeror shall be capable of self-performing at least the minimum of 15%. The contract resulting from this solicitation is not intended for management firms. OFFERORS SHALL COMPLETE THE ATTACHED CALCULATION OF SELF-PERFORMED WORKSHEET AND SUBMIT THE COMPLETED FORM WITH THEIR PROPOSAL. Additionally, offerors must be certified by the Center for Veterans Enterprises (CVE) upon submission of proposals and at the time of award. THE PROPOSAL: The proposal must set forth full, accurate and complete information as required by the solicitation. The Government will rely on such information for the award of the contract. By submission of the offer, the Offeror agrees that all items proposed (if applicable e.g. key personnel, subcontractors, safety plan, etc.) will be utilized for the duration of the contract and any substitutions will require prior Contracting Officer's approval. The offeror shall be held responsible for the validity of all information supplied in the proposal, including the information provided by potential subcontractors. Should subsequent investigation disclose that the facts and conditions are not as stated, the proposal may be rejected. STATEMENT OF WORK Background: The 40 year old floor has formed large bubbles which cause tripping hazards, infection control concerns and was under I.G. review in 2015. The facility replied to the I.G. that the floor would be corrected if funds were available in 2016. Scope of Work: The contractor will furnish all materials, equipment, supervision, and personnel necessary to accomplish the project as indicated on the drawings and comply with VA specifications and regulations. The project will involve, but is not limited to existing floor covering removal, preparation of sub floor, installation of seamless flooring and welding of seams. The final bid must include: The contractor will provide all materials, flooring, tools and labor to complete work, when and as described. The contractor must obtain FORBO Certification prior to the start of this project. The contractor must be proficient at flooring installations. The contractor selected will provide a diagram of the direction in which the sheet vinyl and hall accents (mentioned below) will be installed prior to starting any work. It will be presented and reviewed for approval with the COR. His diagram will not impact the square foot of material and labor required. It is required to assure any practical and visual preferences to the VA. All installation work is to be started and completed in increments of 72 hour (3 days) weekends to include night hours (4PM to Midnight). Any existing VAT will be undisturbed and covered by the new flooring. There is no need for asbestos abatement. The contractor will prepare the area and perform all work as directed by Infection Control which will include blocking HVAC returns, taping door frames, providing sticky mats and any other recommendation from Infection Control that contain dust, odor and fumes. When the 3-day weekend schedule is established after the bid, the COR may request and allow the contractor to perform labor on other weekdays the operating room is not in use, if the contractor is able and willing to complete the job sooner. The bid must still be based on weekend and evening hours. Location: Building 200, 1st Floor: 5 Operating Rooms, Storage Room, the Core and Adjacent Hallways (See Diagram). All locations specified are connected and in the same vicinity, estimated square footage 4,200 square feet. The location has been divided into three areas, each area will be completed and verified and accepted by the COR before work can begin on the next area. 1st Location: OR Rooms 1, 3, 4, 5 - 1,570 square feet (See Floor Plan) 1.Rip up and disposal of existing sheet vinyl and flash covering 2.Floor preparation with double skim coat and install FORBO Marmoleum Sheet (#3032 Mis Grey) to include 6" Flash Covering and Heat Welding of all seams. 2nd Location: OR Rooms 2, OR Storage Room and OR South East Connecting Corridors = 1,595 square feet 1.Rip up and disposal of existing sheet vinyl and flash covering. 2.Floor preparation with double skim coat and install FORBO Marmoleum Sheet (#3032 Mis Grey) to include 6" Flash Covering and Heat Welding of all seams. 3rd Location: OR Inner Core Area = 950 square feet 1.Rip up and disposal of existing sheet vinyl and flash covering. 2.Floor preparation with double skim coat and install FORBO Marmoleum Sheet (#3032 Mis Grey) to include 6" Flash Covering and Heat Welding of all seams. Scheduling: The work shall begin within 15 calendar days of award, unless otherwise agreed upon. Scheduling for this job is between 3PM and 6AM. The contractor must include and bid to include 5 entire weekends plus one day each weekend equal to 72 hours (which include five (5) Saturdays and Sundays). All labor rates/differentials to cover these hours MUST be included and stated in the bid and must specify that element in the bid by interested contractors. The contractor will complete the entire job in these 6 x 72 hour weekends. The contractor selected must provide references that demonstrate they are capable of completing this work properly and on time. Materials: Contractor supplies all materials, adhesives, leveling compounds, tools and labor for night and weekend installation to be completed in 6 x 72 hour time frames. FORBO adhesive S1195 High Moisture Adhesive will be used for this job, which the contractor will provide. FORBO Marmoleum Sheet (#3032 Mist Grey) will be ordered and provided by the contractor. Any reasonable overages to complete the job, since sheet vinyl comes in rolls, will be at the expense of the contractor. The contractor must account for these in the bid.
