- Attachment 4 - Wage Determination Attachment 3 - Financial Responsibility Form Attachment 2 - NAF Electronic Payment Form Attachment 1 - Statement of Work Request for Quote The United States Air Force Academy (USAFA) Fitness & Sports Center has a NAF service requirement for a Group Exercise Class Instructor for a base plus four (4) option years in accordance with the Statement of Work (attachment 1). The awarded contract will consist of a base plus four (4) one (1) year option periods. Periods of Performance: Base POP 1 Oct 16 - 30 Sept 17, Option year one (1) POP 1 Oct 17 - 30 Sept 18, Option year two (2) POP 1 Oct 18 - 30 Sept 19, Option year three (3) POP 1 Oct 19 - 30 Sept 20, Option year four (4) POP 1 Oct 20 - 30 Sept 21.
