Description
The contractor shall provide all labor, materials, equipment and supervision required to complete Project Number 541-16-101, "Improve Water System to Prevent Legionella" at the Department of Veterans Affairs Louis Stokes Medical Center, 10701 East Boulevard, Cleveland, Ohio 44106-1702. The project will upgrade and correct the facility's Potable Water System to reduce the potential for formation and growth of Healthcare-associated Legionnaire's Disease (HCA-LD) and other contaminants, as well as ensure fixtures are up to date for anti-scald prevention. Project will address recommendations made by the Facility Water Safety Management Plan/Committee and bring the facility into compliance with VHA directive 1061 (Prevention of Healthcare-Associated Legionella Disease and Scald Injury from Potable Water Distribution Systems) by addressing the following items: Dead legs, anti-scald, system monitoring, circulation and recirculation, documentation of existing conditions and investigation into possible Biocide Treatments. All work shall be conducted in complete accordance with project drawings and specifications. All work to be closely coordinated with the VA Contracting Officer (CO) and Contracting Officer's Representative (COR). DURATION OF PROJECT: All work to be completed within 500 calendar days from date of receipt of Notice to Proceed (NTP). SOLICITATION DOCUMENTS: The solicitation documents will be posted at www.fbo.gov on or about August 15, 2016 with bid opening approximately 30 days later. Dissemination of the solicitation, specifications and drawings is limited to electronic medium. All interested Bidders (primes & subcontractors) should register at www.fbo.gov so that others will know of your interest in participating in this procurement and for automatic receipt of updates or amendments. A Bidder's mailing list WILL NOT be prepared nor distributed by the Contracting Officer, as it is the Bidders responsibility to register at the www.fbo.gov website. SET-ASIDE INFORMATION: This solicitation will be issued as a 100% Set-Aside for Small Business (SB). The SB must be small under the relevant North American Industrial Classification Standard (NAICS) code and MUST be registered in www.sam.gov to be eligible for award. Bidders MUST ensure registration in the website is accurate, complete and have not expired prior to bid opening date. Project magnitude is between $1,000,000 and $2,000,000. The NAICS code for this project is 236220 and the business size standard is $36.5 million. BONDS: Bonding in the form of Bid, Payment and Performance will be required. SITE VISIT: A one-time only site visit will be scheduled. That information will be in the solicitation package. BID Opening: The bid opening for this project will be at the Network Contract Office 10 at 3140 Governor's Place Blvd. Suite 210, Kettering, OH 45409. SOLICITATION AMENDMENTS: Amendments to the solicitation will be posted at www.fbo.gov. Paper copies of the amendment will NOT be individually mailed. By registering to "Watch This Opportunity", you will be notified by e-mail of any new amendments that have been issued and posted. No other notification of amendment(s) will be provided. Bidders are reminded that they are responsible for obtaining and acknowledging all amendments to this solicitation prior to the time for receipt of offers as prescribed by FAR 52.214-3, Amendments to Invitation for Bids. Bidders should re-visit the website periodically to look for updates. All requests for information (RFIs) are required to be in writing to the Contracting Officer and shall be received within twelve (12) days prior to bid opening; no telephone inquiries will be accepted. It is the interested bidders' responsibility to be sure your questions were actually received in the contracting office for a response, prior to the twelve (12) day cut off period. Questions received after the cut off period will not be responded to. Questions shall be emailed to matthew.curtis1@va.gov. The Contracting Officer cannot guarantee that a response to e-mailed questions will be received.
