- Center for Healthcare Organization and Implementation Research (CHOIR), VA Boston Healthcare System has a requirement for the administration of a patient survey to Veterans. This is not a solicitation announcement. This is a sources sought synopsis only. The purpose of this synopsis is to gain knowledge of potential qualified sources and their size classification (large business, small business, 8(a), SDVOSB, etc.), relative to NAICS code 541611 (size standard of $15M). Responses to this synopsis will be used by the Government to make appropriate acquisition decisions. Based on responses to this synopsis, the Government may issue an Request for Quote (RFQ ). Background: The VA Patient-Centered Care Survey (VA PCC) is designed to assess unique aspects of patients' perceptions of patient-centered VA care in ambulatory and hospital settings. The survey will be administered to approximately 5,000 Veterans. The survey is to be prepared, printed/photocopied and mailed to approximately 5,000 potential Veteran subjects. Approximately half of the Veteran subjects will have been hospitalized within the past month (about 2,500 inpatients) and the other half will have had outpatient visits in the past 6 months (about 2,500 outpatients). There will be two versions of the survey with questions customized for each patient group (inpatient or outpatient). Both versions of the survey booklet will be about 14 pages. The study will use paper surveys only; there is no web survey option. Please assume an overall response rate of approximately 33% (about 1,650). This project will have up to 4 mail contacts with each Veteran subject. The 4 potential contacts include a mailed pre-survey notification letter, the survey booklet with a cover letter, study fact sheet and opt-out postcard, a thank you/reminder postcard, and a second survey to all non-respondents. With multiple mailings (initial and follow-up to non-respondents), this project requires frequent, often daily, contact and coordination between the contractor and CHOIR. Experience in other studies has underscored the desirability and necessity of having a dedicated contractor account representative/project manager for the highly detailed management of the project. In addition to preparation of the survey materials the contractor will be responsible for data entry of the returned surveys and provision of regular status reports about incoming surveys. Performance Period: Date of Award, plus one year Type of Contract: Firm Fixed Price This notice is not a request for competitive proposals, however; any responsible source who believes it is capable of meeting the requirement may submit a capability statement to the contracting office no later than Thursday, August 4, 2016, 12:00 Noon, EST. Interest/capability statements may be sent to Elaine DiBucci at elaine.dibucci@va.gov and should include company name, address, point of contact, business size, product information and GSA Contract Number if applicable. No telephone responses will be accepted. This notice is to assist the Government in determining sources only. A solicitation is not currently available. If a solicitation is issued, all interested parties must respond to that solicitation announcement separately from the responses to this announcement.
