Please return completed Solicitations to me at: CAROLYN.GEORGE@DLA.MIL On this Written Solicitation the following sections require responses in addition any other pertinent areas that should be filled out: *Please sign page 1 fill in blocks 17a, 30a, 30b, 30c. (Standard Form 1449) * Insert Unit Price * Insert Amount *Update Delivery Date if applicable * Two questions are listed here under the section that begins with: (2) The Offeror represents that ” Answer appropriately. Tax and Criminal Information. (Clause 52.209.11) * Clause 52.212-3 see sections: C 1-5, H 1-4 Answer appropriately. (This Clause is quite a few pages long.) * Update Certification Regarding Actual Manufacturer (Business Size Identification) Section 1 with the name and address of manufacturer. Be sure to list the complete address. (Country of Origin). Be sure to list your cage code and the country of manufacturer. Incomplete Information from the pages mentioned above will delay the process and your quote might be considered Unresponsive due to lack of information. Return completed; Written Solicitation to Carolyn George either by email: Carolyn.George@dla.mil or by Fax: 360-476-4121. (If faxing please call or email me so that I can be on the look-out for your faxed quote.) Our department is now doing Written Solicitation for all Quote Request.
