- Attachment 7, DPM Invoice template Attachment 6, Pricing Schedule Attachment 5, Contractor Information Attachment 4, SF LLL Disclosure of Lobbying Activities Attachment 3, Insurance certification Attachment 2, Wage Determination Attachment 1, PWS dated 2 June 2016 FA4830-16-R-0006, Packing and Crating Solicitation Packing and Crating Services Moody Air Force Base, GA. intends to issue a solicitation for non-personal services for all personnel, equipment, tools, supervision, and other items necessary to ensure that Packing and Crating Services are performed at Moody Air Force Base (MAFB) and surrounding areas. The solicitation will close on 14 August 2016 at 1:00 Pm EST. Please submit all questions to michael.mcfadden.16@us.af.mil and 23CONS.LGCB@us.af.mil no later than 22 July 2016 at 1:00 PM EST. The selected/awarded Contractor shall : Furnish all personnel, equipment, facilities, supplies, services and materials, except as specified in the PWS as Government-Furnished, for the preparation of personal property of military members and civilian employees for movement or drayage and related services. The contractor shall provide continuous, efficient, and cost effective service. The contractor shall provide or be responsible for having personnel with proper certification, qualifications, and licenses. The contractor shall maintain facilities in a neat, clean, and orderly condition at all times. The contractor is encouraged and expected to use innovative approaches to efficiently and effectively accomplish requirements in a timely manner in a way that fosters pride and ownership in the work performed. Please see attached draft PWS for more specification of the work to be performed. The solicitation number for this acquisition is FA4830-16-R-0006; it will be issued as a Request for Proposal (RFP). This solicitation will be set aside for small business concerns with a $27.5 million Small Business Size Standard. The solicitation will be issued on or about 27 May 2016. Performance is to begin on 18 September 2016 with a base year and four (4) option years. The primary North American Industry Classification System code is 488991. The Federal Supply Classification code is V301. The solicitation will be available to view and download on the Federal Business Opportunities website ( www.fedbizopps.gov ). Paper copies of this solicitation will not be available. All prospective offerors are responsible to visit the web page frequently and obtain the solicitation, any amendments, and/or other information pertaining to this solicitation.
