- Solicitation Package DD254 Form that needs to be filled and submitted with the Solicitation Package Enclosed is a Request for Quotations (RFQ) for preventive maintenance services for the fuel systems. If you would like to submit a quotation, follow the instructions in Section 3 of the solicitation, complete the required portions of the attached document, and submit it to the address shown on the Standard Form 1449 that is part of the Solicitation package. The U.S. Government intends to award a contract/purchase order to the responsible company submitting an acceptable quotation at the lowest price. We intend to award a contract/purchase order based on initial quotations, without holding discussions, although we may hold discussions with companies in the competitive range if there is a need to do so. In adherence to requirements stipulated by U.S. Government Federal Law, FAR provision 52.204-7, vendors that enter into business with the US Government must be registered in the System for Award Management (SAM) database to obtain a Data Universal Numbering System (DUNS) number https://www.sam.gov/portal/public/SAM/ Quotations are to be sent to Contracting Officer, US Embassy Suva, 158 Princes Road, Suva or by email at GSO_Suva@state.gov. The DD254 form has been included in this Soliciation package. Include the Request for Quotation number SFJ60016Q0003 in the subject line. All quotations are due by 12 noon local [Suva] time August 16th 2016.
