Description
Network Contracting Office 16 intends to procure a Dental Laboratory contract for the fabrication of dental appliances. This contract is to augment the Veterans Healthcare System of the Ozarks current Dental Lab located in Fayetteville, Arkansas. The Lab is currently fully operation but not fully staffed - this contract seeks to provide about 20 to 25% of the current workload. This procurement is set-aside for Veteran Owned Small Businesses. The contract will be for a base year and four (4) 1-year option periods. The proposed statement of work is attached. To obtain a copy of the solicitation, interested vendors should provide Certification of their Veteran Owned status from the Vendor Information Pages at https://www.vip.vetbiz.gov/. This certification will also provide Firm Name:, DBA:, State:, DUNS:, Location:, Phone:, Email:, Expiration:. Firms awaiting certification should provide evidence that the application has been submitted along with the other information which would be on the certification. Interested vendors should also submit a capability statement outlining their ability to provide dental laboratory services. STATEMENT OF WORK The Veterans Health Care System of the Ozarks (VHSO) Dental Service is requiring services for fabrication and repair of Dental Fixed and Removable Prostheses. The VHSO maintains an on-site Dental Laboratory but is unable to fully staff it. Approximately 20 - 25% of the workload will be performed by the Contractor. The Contractor shall provide all resources necessary to fabricate and repair Dental Fixed and Removable Prostheses which the VHSO Dental Service directs to be performed via the contract, delivering in a manner specified by the Schedule for Deliverables. A.General Requirements: I.Primary Laboratory Services Contractor shall provide the following dental laboratory services to the VHSO Dental Service. The Contractor shall provide all materials and supplies necessary to complete each item. For every task listed in the solicitation, the Contractor shall identify in writing all necessary subtasks (if any) and associated costs. Service(s) that is/are not listed in this solicitation must be added as a fixed price Service Line Item Number (SLIN). All contract modifications must be completed by the VA Contracting Officer. 1.Fabrication of Valplast Partials 2.Repairs of Valplast Partials 3.Fabrication of Cast Metal Partials 4.Repair of Cast Metal Partials 5.Fabricate Crowns for Implant Abutments 6.Fabrication of Gold and Porcelain Crowns 7.Fabrication of Removable Complete Dentures and Partial Dentures II.Augmented Laboratory Services VHSO Dental Service may request augmented services. The Contractor shall provide all materials and supplies necessary to complete each item. Contractor shall provide the following dental laboratory services to the VHSO Dental Service. For every task listed in the solicitation, the Contractor shall identify in writing all necessary subtasks (if any) and associated costs. Service(s) that is/are not listed in this solicitation must be added as a fixed price Service Line Item Number (SLIN). All contract modifications must be completed by the VA Contracting Officer. 1.Fixed Prosthetic Crowns (precious, non-precious, Porcelain fused to Metal (PFM) Ceramic) 2.Traditional acrylic partials with wire clasps 3.Crowns for implants 4.Splints 5.Mouth Guards 6.Processed Hard and Soft Relines or Rebases 7.Porcelain fused to metal crowns and bridges 8.All gold crowns and bridges 9.All ceramic crowns and bridges 10.Processed provisional crowns and bridges 11.Diagnostic wax-up and design services for removable and fixed prosthodontics 12.Pre-fabricated and custom abutments for implants 13.Implant supported crowns and bridges 14.Maryland bridges 15.Precision milled retentive bars 16.Precision and semi-precision attachments 17.Cast gold post and cores 18.Partial denture frameworks 19.Implant supported complete and partial dentures III.Relines and Repairs The Contractor shall provide all materials and supplies necessary to complete each item. Acceptable supplies for removable prosthodontics include: Lucitone 199 denture acrylic and Dentsply Portrait denture teeth. Acceptable supplies for fixed prosthodontics include: type 3 gold, noble metals and quality porcelain systems such as IPS Empress, Procera, and Zirconia. B.REQUIRED CRITERIA The Contractor shall provide items that meet the quality, esthetic and functional requirements of the patient. All dentures and partials will carry a patient identification. The Contractor shall be familiar with mould guides by Dentsply Portrait and with shade guides by Dentsply Portrait. Materials utilized shall be from ISO 9001/CE registered companies. The Contractor shall use procedures, techniques, and current technology, which are considered to meet Federal Drug Administration (FDA) regulations and American National Standards Institute (ANSI) guidelines. The Contractor shall not subcontract any of VHSO dental laboratory services for the duration of the contract. The following materials shall be used in the fabrication of dental prostheses: Complete Dentures/Removable Partial Dentures Lucitone 199 acrylic resin Color coloration or equivalent Occlusal Scheme: Monoplane, linear Anatomic, full balanced Lingualized, linear 33 */monoplane 30 *rational Functional/rational Tooth material: Dentsply Portrait The Contractor shall at no additional expense to VHSO, re-make all dental items that are not correct, cannot be properly seated, or flawed due to Contractor error. If, in the case of porcelain work, the porcelain shatters within twelve months of fabrication, it shall be considered flawed and remade at no additional expense to VHSO. The Contractor's laboratory shall provide: 1.Case consultation with individual VHSO Dentists 2.Responsive and knowledgeable customer service representatives at no cost 3.Patient identification placed on all dentures and partials that consists of initial of last name and last four numbers of social security number 4.Fabrication and repairs conducted in-house/on-site 5.No charge for daily pick-up, tracking and delivery services 6.Oversight and administration of pick-up and delivery services 7.Establish and maintain a record keeping system that will track deliveries to VHSO Dental Service and the return of services requested from the Contractor's location of service 8.Detailed Personalized Prescription Pads 9.Shipping containers, packing materials and shipping labels 10.Adherence to The Centers for Disease Control (CDC) infection control guidelines for all incoming and outgoing cases. 11.A Quality Assurance Program: Quality Assurance Process (QA) that will guarantee a product that meets the patient's natural esthetics & superior fit with minimal chair time. Use of materials purchased from ISO 9001/CE registered companies. No lead products or by-products will be used in any fabrications or repairs. Contractor shall perform internal technical audits to ensure consistent, high quality, and timely restorations. The Contractor's laboratory shall use procedures, techniques, and current technologies, which are consistent with Occupational Safety and Health Administration (OSHA) standards. 12.All prostheses will be fabricated in the United States of the America using materials approved by the Food and Drug Administration (FDA). C.SCHEDULE FOR DELIVERABLES The Contractor shall provide blank prescription forms to VHSO Dental Service to be completed by the prescribing dentist. VHSO Dentists will provide dental service prescriptions, accurate impressions and bite registrations. The form shall include the following information: I.Initial of the last name, Last 4 digits of patients Social Security number, Last 2 digits of the birth year (@####-##) II.Type of prosthesis to be fabricated and any additional instructions III.Attending dentist IV.Date prosthesis sent V.Return Date VI.Special instructions If a prescription is not legible or clearly understood, the contractor must contact the VHSO Chief or treating dentist to obtain a clear and legible prescription. Impressions, teeth, prescriptions, finished product, etc. shall: I.Be picked up from VHSO Dental Service daily with delivery/arrival to Contractor's laboratory within twenty-four hours or shipping boxes and pre-paid postage labels with next day service will be provided by the contractor. II.Completed work shall meet the Schedule of Deliverables for the laboratory services listed below and returned/delivered to VHSO Dental Service within twenty-four hours. The completion working days commence upon the twenty-four hour receipt of requested services. III.Daily pick-up and deliveries shall be accomplished Monday through Friday (working days), 9:00 a.m.-3:00 p.m. (working hours), excluding Federal Holidays and any other day specifically declared by the President of the United States to be a national holiday unless overnight mail service is used. 1.Primary Laboratory Services-- shall be completed no later than ten (10) working days from the day of arrival at the laboratory unless agreed upon with the VHSO Dentist(s), VHSO designee, or Contracting Officer. If timeframes cannot be met, the Contractor shall give notice to the VHSO Dentist(s), VHSO Dental Service, VHSO designee, or Contracting Officer for rescheduling patients. 2.Augmented Laboratory Services-- shall be completed no later than ten (10) working days from the day of arrival at the laboratory unless agreed upon with the VHSO Dentist(s), VHSO designee, or Contracting Officer. If timeframes cannot be met, the Contractor shall give notice to the VHSO Dentist(s), VHSO Dental Service, VHSO designee, or Contracting Officer for rescheduling patients. 3.Relines and Repairs-- shall be completed no later than four (4) working days from the day of arrival at the laboratory unless agreed upon with the VHSO Dentist(s), VHSO designee, or Contracting Officer. If timeframes cannot be met, the Contractor shall give notice to the VHSO Dentist(s), VHSO Dental Service, VHSO designee, or Contracting Officer for rescheduling patients. D.QUALIFICATIONS: The Contractor shall furnish an up-to-date list of all certified/licensed Dental Laboratory Technicians and Dentists that are assigned laboratory work from VHSO Dental Service. The list of certification/licensure shall be updated every six months and provided to the VHSO Contracting Officer's Technical Representative as needed and/or certified as accurate and complete. I.The Contractor's laboratory shall be certified by the National Association of Dental Laboratories. A current copy of certification shall be provided to VHSO Contracting Officer's Technical Representative. II.Personnel assigned by the Contractor to perform the services covered by this contract shall be certified dental laboratory technicians by National Association of Dental Laboratories (NADL) or other similar certification by a certified or recognized training institution. The Contractor shall provide certification/licensure to the VHSO Contracting Officer's Representative as needed and/or certified as accurate and complete every six months. E.CHANGES TO STATEMENT OF WORK: Any changes to this Statement of Work (SOW) shall be authorized and approved only through written correspondence from the Contracting Officer. A copy of each change will be kept in a project folder along with all other products of the project. Costs incurred by the Contractor through the actions of parties other than the Contracting Officer shall be borne by the Contractor. F.HEALTH INSURANCE PORTABILITY AND ACCOUNTABILITY ACT (HIPAA): The Contractor shall comply with all applicable privacy and confidentiality Statutes and Regulations. HIPAA requirements contractor must adhere to the provisions of Public Law 104-191, Health Insurance Portability and Accountability Act of 1996 and National Standards to Protect Privacy of Protected Health Information (PHI). G.INVOICING Invoices must be submitted monthly and contain at a minimum the following information: I.Purchase Order number II.Vendor Name, address, phone number, tax ID number III.Initial of the last name, Last 4 digits of patients Social Security number, Last 2 digits of the birth year (@####-##) IV.Cost per unit, Cost of metal per case V.Specific type of metal used in each prosthesis in accordance with the American Dental Association (ADA) VI.Total Cost VII.Date of Service All requests for payment shall be submitted via the FSC c-Invoice Program (see http://www.fsc.va.gov/einvoice.asp) Invoices must be available upon request to validate the number of completed VHSO dental laboratory cases, cost per case and total cost per month. The Contractor must maintain invoices throughout the duration of the contract. H.PERIOD OF PERFORMANCE The Period of Performance for this contract is from August 1, 2016 through July 31, 2017 with four renewable option years, if exercised by the government. The contract is subject to the availability of funds. I.WORK HOURS The services covered by this contract shall be furnished by the Contractor as defined herein. Working Days/Working Hours: Monday through Friday, 8:00 a.m. - 4:30 p.m. The Contractor shall not be required to furnish emergency services or services during off-duty hours as described below. National Holidays as observed by the Federal Government are: I.New Year's Day II.Martin Luther King's Birthday III.Presidents Day IV.Memorial Day V.Independence Day VI.Labor Day VII.Columbus Day VIII.Veterans Day IX.Thanksgiving X.Christmas And any other day specifically declared by the President of the United States to be a national holiday. J.RECORD KEEPING The Contractor shall establish and maintain a record keeping system that will track deliveries to VHSO Dental Service and the return of services requested from the Contractor's location of services. K.CONTRACT PERFORMANCE The VHSO COR will monitor the Contractor's performance to ensure contract compliance, and to ensure VHSO is receiving the following services: I.TIMEFRAME - The Contractor shall complete and return all dental laboratory work delivered to the Contractor's service location per Schedule of Deliverables. The Chief, Dental Service or his designee is VHSO's appointed official responsible for contract compliance. After contract award, any incidents of Contractor noncompliance as evidenced by these monitoring procedures shall be forwarded to the Contracting Officer for review. II.INVOICE VERIFICATION -The Contractor shall provide a complete and accurate invoice each month with each case. A copy of each invoice will be mailed to Dr. Thomas R. Rhyne for verification and certification of accuracy prior to being submitted for payment. Each completed case will be identified by the service provided on the invoice. III.CASE CONSUL TATION -The Contractor shall provide case-by-case consultation as needed on provided services within twenty-four hours of consultation request. IV.FLAWED PRODUCT -The Contractor shall rework flawed products, which were not done by them within the same timeframe as other products delivered to them for laboratory services. L.CONTRACT AWARD MEETING The contractor shall not commence performance on the tasks in this SOW until the Contracting Officer has conducted a post-award conference, or has advised the contractor that a post-award conference is waived.
