Solicitation Number : W912K6‐16‐Q‐0062 Notice type: Combine Synopsis/Solicitation Response Date : August 11, 2016, 10:00 AM, EST. Set Aside: Total Small Business/GSA Schedule Offers Only Classification Code : 65 - Medical, Dental, and Veterinary equipment and Supplies NAICS Code : 541990 - All Other Professional, Scientific and Technical Services Subject : The scope of this project is for a yearly maintenance and replacement of Automatic External Defibrillators (AED) at the end of Life cycle for Maryland Army National Guard (MDARNG) throughout all military installations. Purpose: Maryland Army National Guard proposes to enter into a Firm‐fixed Price contract for AED yearly maintenance and replacement at the end of the Life cycle. Synopsis: " PLEASE READ THIS NOTICE CAREFULLY AS IT CONSTITUTES THE ONLY NOTICE THAT WILL BE ISSUED." This is combined synopsis/solicitation for commercial items prepared in accordance with the format prescribed in the Federal Acquisition Regulations (FAR) subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Solicitation W912K6‐16‐Q‐0062 is issued as a request for quotation (RFQ). The results of this requirement will be a Firm‐Fixed priced (FFP) contract. The right to make multiple or no award is reserved in the event it is advantageous to the Government to do so. The RFQ is being conducted as a 100% small business set‐aside under GSA Schedule only. This RFQ incorporates provisions and clauses in effect through Federal Acquisition Circular 2005‐88. This requirement will be awarded on an all or none basis as a commercial item Fixed Price purchase order under procedures on FAR Parts 12 and 13. Offer/quote must be valid for 30 days. New Equipment ONLY, NO remanufactured or "gray market" items. All items must be covered by the required manufacturer's warranty. The Contractor shall extend to the Government full coverage under a normal commercial warranty. Acceptance of the warranty does not waive the Government rights with regard to the other terms and conditions of this contract. The Warranty Period shall begin upon final acceptance of all items provided to the Government by the Contractor. This Solicitation is for a Base Year and four (4) Option years, and options to include equipment at the end of their life cycle. Contract Type and Evaluation Criteria : Award will be made to the responsive and responsible offeror whose offer results in Lowest Price Technically Acceptable. The North American Industry Classification System (NAICS) is 541991 All Other Professional, Scientific and Technical. The small business size standard is $15.0 million Dollars. Destination/Place of Performance : See attached AEDs Location List Delivery/Performance: Ten (10) Days after receipt of order (ARO). Acceptance Criteria: AED replaced will be free of damage and fully operational after delivery. No partial shipments unless otherwise specified at the time of order.
