- DRAFT Performance Work Statement (PWS) The Mission Installation Contracting Command Center-Fort Knox is conducting market research for Mortuary Services for the Human Resource Command. The market search is to determine the capabilities of all businesses (large and small) interested in the opportunity described below. THIS SOURCES SOUGHT ANNOUNCEMENT IS NOT A REQUEST FOR PROPOSAL AND THE GOVERNMENT IS NOT COMMITTED TO AWARD A CONTRACT PURSUANT TO THIS ANNOUNCEMENT. It is a market research tool being used to determine a procurement strategy. The Government is not obligated to, and will not, pay for any information received from potential sources as a result of this notice. The applicable NAICS code is 812210 and the size standard is $7,500,000.00. This contract requirement has a base period and four option periods. Base period of performance is 20 May 2017 through 19 May 2018. The Army has a requirement to provide mortuary services to United States Army Soldiers that have been classified as Deceased within CONUS, including Alaska, Hawaii, and Puerto Rico. The deceased Soldier's remains are transported to a servicing mortuary for embalming, dressing, cosmetic work, and shipment to another mortuary/cemetery for final burial. Shipment is worldwide. The contractor shall provide all personnel, equipment, supplies, facilities, transportation, tools, materials, supervision, and other items and non-personal services necessary to perform mortuary services (care and disposition of human remains) and disinterment services as defined in this Performance Work Statement (PWS), except for those items specified as Government furnished property and services. The contractor shall perform to the standards in this contract. The contractor shall provide facilities, equipment, licensed personnel, motor vehicles, professional services, supplies, death certificates, any/all required documentation for OCONUS shipments, caskets, urns, outer shipping containers, transportation, and perform hygienic practices as specified in this PWS. The contractor shall comply with the highest industry standards in fulfilling the terms and conditions of this contract. Your response shall be no more than five pages in lenght and include the following: 1. Business Information a. Company name and address b. D&B Duns Number c. Socio-economic classification of company (i.e., small business, 8(a), Woman Owned, Service Disabled Veteran Owned, HUBZone, Large business, etc.) d. Size (gross revenue, number of employees in company) e. Number of years in existence f. Company point of contact - name, phone, and email address 2. Vendor's Capabilities Descriptions of the vendor's capabilities to provide the mortuary service support consistent with the duties listed in this announcement and attached Performance Work Statement (PWS). Responses to this announcement are due at 4:00PM, Eastern Standard Time, on Friday, 8 July 2016, must be in writing, and shall be sent electronically to Robert Woolsey at robert.l.woolsey2.civ@mail.mil.
