- SOURCES SOUGHT - FOH EMPLOYEE ASSISTANCE PROGRAM (EAP) This is a Small Business Sources Sought notice. This is NOT a solicitation for proposals, proposal abstracts, or quotations. The purpose of this notice is to obtain information regarding: (1) the availability and capability of qualified small business sources; (2) whether they are small businesses; HUBZone small businesses; service-disabled, veteran-owned small businesses; 8(a) small businesses; veteran-owned small businesses; woman-owned small businesses; or small disadvantaged businesses; and (3) their size classification relative to the North American Industry Classification System (NAICS) code for the proposed acquisition. Your responses to the information requested will assist the Government in determining the appropriate acquisition method, including whether a set-aside is possible. An organization that is not considered a small business under the applicable NAICS code should not submit a response to this notice. Questions regarding the contents of this Sources Sought shall be submitted via email to: saundra.kpadeh@psc.hhs.gov no later than 2:00pm EST on July 11, 2016. Phone calls will not be accepted. The Government reserves the right to respond to some, all, or none of the questions that will be submitted. Capability statements and responses shall address all of the information listed under "Capability statement/information sought". Capability statements and responses shall be submitted via email to Saundra.Kpadeh@psc.hhs.gov no later than 2:00pm EST, July 28, 2016 in the form of a "white paper." The subject line for the email heading shall be "EAP and Work/Life Program Services"- (enter the Source Sought reference number). Responses should be limited to no more than ten (10) single spaced pages exclusive of marketing slicks or published information available at time of release of the Sources Sought request. Page size is 8.5 x 11 inches with 1-inch margins. Text must be a minimum font size/style of 10-point Times New Roman. Tables must be a minimum font size/style of 9-point in the Arial family. Graphics must be at minimum 8-point in the Arial family. Tables and Graphics may use the landscape format. All other text must use the portrait format. See attached Sources Sought.
