- CORRECTION MADE TO INSERT CORRECT PCB CONTAMINATED EQUIPMENT LIST. NOTE: Amendment A001 is to provide: Response to vendor ™s questions. Clarify that an Oklahoma DOL Contractor License for Asbestos Abatement is NOT required. Extend the proposal closing date to 2:00 pm, July 18,2016. Changes period of performance from 180 days to 60 days after notice to proceed. List of PCB containing equipment. This is a request for quote for PCB Contaminated Equipment Removal located at the FAA Mike Monroney Aeronautical Center (MMAC), Oklahoma City, OK. Vendor will provide all labor, equipment, vehicles and materials needed to remove and properly load transport and dispose of 500 PPM or more Polychlorinated Biphenyl (PCB) oil-filled equipment that has a weight total of 153,400 pounds on approximately 118 pallets. Projected start date is 30 days after award. The FAA will not pay for any information received or costs incurred in preparing the response to the request for quote. Therefore any cost associated with the request for quote submission is solely at the interested vendor's expense. This competition is open to large and small businesses. Interested firms should review the attachments and provide a quote and the required documentation to bill.white@faa.gov no later than 2:00 p.m. June 30, 2016. The Offeror ™s proposal must include a copy of their Oklahoma Department of Labor Contractor License to conduct asbestos abatement activities and a completed FAA Business Declaration form. Required documentation includes vendor signed page 1, completed page 2 (Section B); page 8 (H.1 “ Place of Disposal), and Section K. All responses to this request for quote must be received by Thursday, June 30, 2016, 2:00 p.m. CT (Oklahoma Time). All submittals should be submitted by email to bill.white@faa.gov. Funds are not presently available for this acquisition. No contract award will be made until appropriated funds are made available. NOTICE for informational purposes for Minority, Women-owned and Disadvantaged Business Enterprises: The Department of Transportation (DOT), Office of Small and Disadvantaged Business Utilization (OSDBU), has a program to assist small businesses, small businesses owned and controlled by socially and economically disadvantaged individuals, and women-owned concerns to acquire short-term working capital assistance for transportation-related contracts. Loans are available under the DOT Short Term Lending Program (STLP) at prime interest rates to provide accounts receivable financing. The maximum line of credit is $500,000.
