Description

This is a Combined Synopsis/Solicitation Notice for Waste Medical Hazardous Services for Salisbury VAMC. This Combined Synopsis/Solicitation Notice is an official request for proposal. Solicitation VA246-16-Q-0627, will be issued on 30 March 2016. The Department of Veterans Affairs Medical Center Salisbury, North Carolina, currently has a need for a contractor to provide all labor, materials, tools, equipment and supervision necessary to provide collection, removal, transportation for the removal of all waste (including Sharps, Pathology Waste and Trace Chemotherapy waste) that is generated by the VA Community Based Outpatient Clinic, Charlotte HCC, NC and Kernersville HCC. 1601 Brenner Avenue, Salisbury, NC 28144, for period of April 11, 2016 through April 10, 2017 with four one year option periods. All waste shall be weighed at site and entered on a Manifest that guarantees tracking of the waste from the premises through the destruction by the Incinerator or Microwave. After the waste is destroyed a copy of the manifest certificate will be sent to the VA (within 30 days from disposal) as proof of the destruction. The NAICS Code is 562112. Size Standard $38.5M. All correspondence and responses shall be via e-mail to Eugene.Coleman2@va.gov and the Contracting Officer Evelyn Ligon-Moton at Evelyn.Ligon-Moton@va.gov.. Facsimile responses will not be accepted. Electronic responses must be received via email no later than 3 PM, April 06, 2016. All contractors interested in doing business with the Government must be registered in the System for Award Management (SAM) database. The website for registration is www.sam.gov. Service-Disabled Veteran-Owned Small Businesses and should also indicate whether the company is verified by the VA in the Vendor Information Pages (VIP) Database at www.vetbiz.gov.
