- 1. This is a combined synopsis/solicitation for commercial items prepared in accordance with the Federal Acquisition Regulation (FAR) format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. 2. Solicitation number NIH-RFQ-4097782A is issued as a Request for Quote (RFQ) using Simplified Acquisitions Procedures in accordance with FAR Part 13 Simplified Acquisition Procedures. 3. This solicitation and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) Number 2005-85-2, dated February 26, 2016. 4. The RFQ is issued as a small business set-aside. The associated North American Industry Classification System (NAICS) Code is 325613 - Surface Active Agent Manufacturing; Small Business Size Standard is 500 in number of employees. 5. The NIH intends to issue a Commercial Item Firm-Fixed-Price purchase order for the purpose of acquiring rock salt in bulk quantities by the ton that satisfy the following requirements: 6. ITEM DESCRIPTION: Rock Salt, for use in road stabilization and ice melting, chemical name is Sodium Chloride (NaCl), total quantity of 400 tons to be delivered 7. DELIVERIES: The rock salt shall be delivered to two (2) separate locations. Delivery Time: within two (2) days after receipt of order Place of Delivery and Acceptance: Three-hundred (300) tons of rock salt shall be delivered to: National Institutes of Health 9000 Rockville Pike Building 13 Platform C Bethesda, MD 20892; and One-hundred (100) tons shall be delivered to: National Institutes of Health 16701 Elmer School Road Building 101A Dickerson, MD 20842 FOB Point: Destination 8. The provision at 52.212-1,Instructions to Offerors-Commercial Items (Oct 2015), applies to this acquisition. The following addenda to the provision is incorporated by reference: 52.204-6, Data Universal Numbering System Number 52.204-7, System for Award Management 52.204-16, Commercial and Government Entity Code Reporting 52.204-17, Ownership or Control of Offeror 52.204-18, Commercial and Government Entity Code Maintenance 9. The provision at FAR 52.212-2 Evaluation-Commercial Items (Oct 2014), is used. (a)The Government will award a purchase order resulting from this solicitation to the responsible offeror whose offer conforming to the RFQ will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: 1) item, 2) price and 2) delivery time. 10. The provision at FAR 52.212-3, Offeror Representations and Certifications-Commercial Items (Nov 2015),applies to this acquisition. An offeror should complete only paragraph (b) of this provision if the offeror has completed the annual representations and certifications electronically at http://orca.bpn.gov. 11. The clause at FAR 52.212-4, Contract Terms and Conditions-Commercial Items (May 2015) applies to this acquisition. 12. The clause at FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items (Feb 2016) applies to this acquisition. 13. Responses: Quotations in response to this RFQ are due on Tuesday, March 1, 2016 by 10:00 a.m EST. Pricing shall include delivery/shipping costs and shall state the contractor's delivery time after receipt of an order.
