Description
The Department of Veterans Affairs Medical Center, NCO 9 Network Contracting Office, on behalf of James H. Quillen VA Medical Center, is conducting market research through this sources sought notice to seek sources that can provide a Mobile Lithotripsy Services. The North American Industry Classification System (NAICS) Code for this acquisition is 621512. Disclaimer and Important Notes: This Sources Sought Notice is for market research purposes only and does not constitute a Request for Proposal/Quotation; and, it is not considered to be a commitment by the Government to award a contract nor will the Government pay for any information provided; no basis for claim against the Government shall arise as a result from a response to this Sources Sought Notice or Government use of any information provided. Failure to submit information in sufficient detail may result in considering a company as not a viable source and may influence competition and set-aside decisions. The Government reserves the right to use any information provided by respondents for any purpose deemed necessary and legally appropriate, including using technical information provided by respondents in any resultant solicitation. At this time no solicitation exists; therefore, Do Not Request a Copy of the Solicitation. After a review of the responses received, a pre-solicitation notice and solicitation may be published on Federal Business Opportunities (FedBizOpps) website. It is the potential offeror's responsibility to monitor FedBizOpps for release of any future solicitation that may result from this Sources Sought Notice. However, responses to this Sources Sought Notice will not be considered adequate responses to any resultant solicitation. Pursuant to FAR Part 10 (Market Research), the purpose of this notice is to: (1) determine if sources capable of satisfying the agency's requirements exists, (2) determine if commercial items suitable to meet the agency's needs are available or could be modified to meet the agency's requirements, and (3) determine the commercial practices of company's engaged in providing the needed service or supply. Pursuant to FAR Part 6 and FAR Part 19, competition and set-aside decisions may be based on the results of this market research. This notice in no way obligates the Government to any further action. SAM: Interested parties should register in the System for Award Management (SAM) as prescribed in FAR Clause 52.232-33. The SAM can be obtained by accessing the internet at www.sam.gov or by calling 1-866-606-8220. Scope of Work: Contractor shall provide Mobile Lithotripsy Services as required to the Department of Veterans Affairs Medical Center; hereafter called James H. Quillen VA Medical Center (JHQVAMC), Mountain Home, Tennessee. The JHQVAMC is located at the Corner of Lamont Street and Veterans Way, Mountain Home, TN 37684. Services shall include one (1) up to two (2) Lithotripsy Technologist(s) (as needed) and Mobile Lithotripsy Equipment. The VA anticipates that an average of five (5) Veteran beneficiaries will be treated per scheduled lithotripsy event. A. SPECIFIC DUTIES AND RESPONSIBILITIES: 1. The Lithotripsy Technologist shall: "Be a U.S. Citizen and have current licensure/certification in good standing within the U.S. "Be registered and in good standing with the American Registry of Radiologic Technologists ® (ARRT)-certified Lithotripsy Technologist. "Be trained by accredited institution a minimum of one year in lithotripsy services and operation of the equipment. "Possess ability to assist attending VA Urologist and other VA providers in the successful operation of mobile lithotripsy equipment provided. "Successfully operate lithotripsy equipment and any related imaging equipment during operating procedures. "Assist as needed, during cases in the training and education of JHQVAMC personnel as it relates to lithotripsy. "Deliver all reports, digital, film and file records of all procedures performed with the equipment to the Contracting Officer's Representative (COR). These medical records shall be the property of the JHQVAMC, Mountain Home, TN. "Maintain current Cardio-Pulmonary Resuscitation (CPR) certification. "Meet U.S. Department of Transportation driver requirements along with a safe driving record. 2. Mobile Lithotripsy services will include, but are not limited to: " Providing portable, state of the art, digital equipment that can be easily moved into the operating room at JHQVAMC. " Obtaining all necessary licenses, approvals, permits and be in compliance with all federal, state and local laws and regulations applicable to the equipment and delivery of lithotripsy related services provided for under the terms of this contract. " Performing all services under this agreement in accordance with any and all regulatory and accreditation standards applicable to the facility, including without limitations, those requirements imposed by The Joint Commission (TJC). Contractor shall provide consultation, if requested, in the development of protocols, utilization review, peer review and risk management as required by TJC and other regulatory bodies. "Additional materials and supplies, including consumables (e.g., electrodes), as are necessary to conduct lithotripsy procedures with the equipment at no additional cost to JHQVAMC. 3. Lithotripsy equipment must meet or exceed the following criteria: "Treats all stones anywhere in the urinary tract. "Have a large focal point to allow for better positioning. "Modular and can easily maneuver around operating suite "Perform endourologic procedures before and after lithotripsy without moving the patient. "Can accommodate patient weight up to 400 pounds. "Must be prepared to perform the first procedure of the day, Monday-Friday, at 8:00am (EST) and continue to perform procedures until JHQVAMC's schedule for the day is completed. "In the event mobile lithotripsy services have to be provided on an urgent or emergent basis during off-duty hours or national holidays, the contractor is required to respond telephonically within 15 minutes of receiving the call and must be in-house within two (2) hours of the original page or telephone call to provide mobile lithotripsy services at JHQVAMC. "Maintenance of the equipment to include service and preventative maintenance to ensure that the equipment is in the highest level of functioning condition. 4. Contractor will be responsible for: "Ensuring Lithotripsy Technologist is self-enrolled in the Talent Management System (TMS) at www.tms.va.gov and notifying the COR when self-enrollment is complete. " Contracted employee annually completes the following mandatory TMS modules: - TMS Module 10203, Privacy and HIPAA Focused Training - TMS Module 10176, VA Privacy and Information Security Awareness and Rules of Behavior. - TMS Module 7318, VHA CO Compliance and Business Integrity (CBI) Awareness Training - TMS Module 3855743, Code Blue Procedures - TMS Module 3836751, Occupational Safety, Health and Fire Protection Program - TMS Module 3823355, Environment of Care: Fire and Life Safety Management - TMS Module 3844813, Bloodborne Pathogens and Tuberculosis Exposure Training - TMS Module 3836886, Emergency Operations - TMS Module 16551, VHA Minimizing Risk for Fluoroscopic X-Ray - TMS Module 3890386, Fire Safety in the OR " Performing all services under this agreement in accordance with any and all regulatory and accreditation standards applicable to facility, including without limitations, those requirements imposed by TJC. " Contractor shall provide consultation, if requested, in the development of protocols, utilization review, peer review and risk management as required by TJC and other regulatory bodies. " Contractor will ensure performance of services in line with the Bylaws and rules and regulations governing JHQVAMC, Mountain Home, TN. "e. Providing post-procedural documentation related to equipment output prior to leaving facility on day of procedure. 5. JHQVAMC will provide the following: "Suitable electric service to operate the equipment; "A secure room for the equipment while located on the premises; "Board certified/eligible Physician(s) to perform the procedure and appropriate licensed "personnel to administer any required anesthesia; "Appointment scheduling for lithotripsy patient procedures and coordination of scheduling of in-service events relating to the equipment for facility staff; "Pre-operative preparation of patients for lithotripsy, obtain written patient "consent forms and provide patients with the necessary services relating to post-procedure "recovery. "Any pharmaceuticals required for the procedures; "Take all actions reasonably necessary to assist contractor in achieving compliance with "federal, state and local laws and regulations. "Shall be responsible for the collection and disposal of infectious waste and sharps related "to the lithotripsy procedures. "JHQVAMC is responsible for providing emergency service personnel, physicians, "facilities and supplies necessary to provide patients with emergency medical "care as may be required, as well as physicians, facilities and supplies necessary to provide "patients with inpatient or outpatient post-procedure care. B. QUALIFICATIONS: Lithotripsy Technologists chosen by the contractor to perform the services covered by this task order must have a current and unrestricted ARRT-certification in a State, Territory, or Commonwealth of the United States or the District of Columbia. Personnel assigned by the contractor to perform the services covered by this task order shall be proficient in written and spoken English (38 USC 7402). The qualifications of such personnel shall also be subject to review by the JHQVAMC Chief, Urology Section, or designee, if required. The Contracting Officer will be notified prior to any changes in personnel. VA reserves the right to approve or disapprove the assignment of individual personnel furnished by the contractor to perform the functions specified in the task order. The personnel assigned by the Contractor must complete VA credentialing process as required. Services shall be provided as soon as possible after notice of contract award and upon completion of all required Credentialing and Background Investigations (as applicable). The Healthcare System will make every attempt to notify the Contractor as soon as possible when procedures are cancelled and/or rescheduled to release the contract employee. In the event of an emergency or unanticipated absence, the notification period will be less and the contractor will be expected to provide coverage within two days of notification. Contractor will make every effort to maintain consistency of assigned staff at the VA at all times for training, security and continuity purposes. Physical Requirements: The contractor must provide documentation that demonstrates that the contractor employee has a current physical that demonstrates ability to perform all aspects of the position, including standing and sitting for extended periods of time, ability to speak English, visual and auditory acuity, etc. Credentials Requested: The Lithotripsy Technologist will be required to complete credentialing during this assignment. An application package will be provided by the VA to be completed by the candidate. This includes: curriculum vitae, current references, signed release of information from candidate, and statement that candidate does not have any physical or mental health condition that would adversely affect his/her ability to carry out assigned duties, and additional credentialing forms. Candidate will also be required to make application through the VETPRO Internet process. Candidate shall need to be enrolled in VETPRO by VA personnel. The website address is: https://fcp.vetpro.org. Point of Contact and Phone Number. 3. Business Size applicable to the NAICS Code: a. HuBZone Small Business Concern; b. Service-Disabled Veteran Owned Small Business Concern (SDVOSBC); c. Veteran Owned Small Business Concern (VOSBC); d Small Business Concern; e. Large Business Concern 4. Documentation Verifying Small Business Certification: a. If claiming HuBZone status, provide a copy of your HuBZone Certificate from SBA. b. If claiming SDVOSBC status, provide documentation that shows the Service Disable Veteran (SDV) unconditionally owns 51 % of the business, controls the management and daily operations, and holds the highest officer position in the company. Documentation should also be provided to show that the SDV has a service-connected disability that has been determined by the Department of Defense or Department of Veteran Affairs. Finally, provide documentation that shows the business is small under the NAICS code 621512. c. If claiming VOSBC status, provide documentation that shows the Veteran unconditionally owns 51% of the business, controls the management and daily operation, make long-term decisions for the business, and holds the highest officer position in the company that works at the business fulltime during normal working hours. The documentation should also show the business is small under the NAICS code 621512. d. If claiming Small Business status, provide documentation to show the business is small under NAICS code 621512.
