- map quote doc and clauses poster This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quotations are being requested. Solicitation AG-52B1-S-16-0007 is being issued as a Request for Quotation (RFQ), to perform tree planting services at the Shawnee National Forest, Jackson county, IL The North American Industry Classification System (NAICS) code is 115310 and the size standard is $7.5mil. This acquisition is set-aside for Small business. Contractors must be H2A compliant. Pricing and technical information will be submitted to: Roger Manning, USDA Forest Service, 811 Constitution Ave, Bedford, IN 47421. NO FAX QUOTES WILL BE ACCEPTED. E-mail quotes are acceptable at rkmanning@fs.fed.us. Service Wage Rates are applicable. Quotes must include vendor's TIN (Tax Identification Number) and DUNS number. Vendors must be registered in SAMS (www.sams.gov) Quotes must include a completed copy of (FAR 52.212-3 Offeror Representations and Certifications, Commercial Items, paragraphs (c) through (m). If the Offeror has completed the annual representations and certifications electronically at http://orca.bpn. gov., submit only paragraph (b) from the provisions and a DUNS number. (FAR) 52.212-2 Evaluation, Commercial Items is applicable with the following factors used to evaluate quotes: Price, technical conformity to specifications, and past performance. Clauses indicated on a separate attachment. A pre-quote meeting will be held Feb. 22 at 1000am local time at the Oakwood Bottoms Green Tree Reservoir Grand Tower, IL. Quotes are due March 2nd, 2016 at 11:00a.m. Eastern Time.
