Description
RFI: Feasibility of Expanding the AHRQ Healthcare Horizon Scanning System Notice Number: HHS-AHRQ-RFI-16-001 Request for Information (RFI): This is a Request for Information (RFI). This is NOT a solicitation for proposals, proposal abstracts, or quotations. The purpose of this RFI is to obtain knowledge and information for project planning purposes. This Request for Information (RFI) is to solicit input with regards to the feasibility of expanding the scope of AHRQ's healthcare horizon scanning system to include innovations and trends that impact health care cost. Background: Horizon scanning is an activity undertaken to identify technological innovations that could have great impact or bring about paradigm shifts. In healthcare, horizon scanning pertains to identification of new (and new uses of existing) pharmaceuticals, medical devices, diagnostic tests and procedures, therapeutic interventions, rehabilitative interventions, behavioral health interventions, health care delivery innovations and public health/health promotion activities. Healthcare horizon scanning programs have long been used by public or private entities around the world for various purposes, including commercial planning, health service research prioritization, financial or operational planning, controlled diffusion of technologies, and provision of information to policy makers, purchasers, and providers of health care. In 2010, AHRQ established the first public horizon scanning system to focus on emerging health interventions and innovations within the unique political, regulatory, cultural, and economic context of the U.S. health care system. The AHRQ horizon scanning system focused on identifying innovations that could have a potential high impact on meeting unmet needs in healthcare (defined as any need arising from a gap in effective ways to screen, diagnose, treat, monitor, manage, or provide or deliver care for a health condition or disease) and improve patient outcomes that is expected to have an impact within the next 1-2 years. Purpose of this RFI: Recognizing rising cost of healthcare and the potential of horizon scanning to forecast future healthcare costs, AHRQ seeks to explore the possibility of expanding the scope of the healthcare horizon scanning system to identify new innovations or new trends in utilization, practice, etc. that have the potential to have a substantial impact on healthcare cost. Examples of these innovations and trends include but are not limited to: • Generic drugs or compounded pharmaceuticals that replace high price drugs and/or other changes in drug pricing paradigms • Changing trends in utilization or in acquisition of major capital equipment such as imaging devices, radiation therapy devices, robotic surgery devices, etc. by health system providers • Changing trends in health services delivery such as: o Movement of site of service for procedures from inpatient to outpatient, hospitals to doctor's offices, in-person care to tele-health, emergency rooms or doctor's office to standalone urgent care centers or centers in pharmacies o Consolidation or merging of providers, changing ownership of hospitals, diagnostic test sites, etc. • Trends in reimbursement such as patient cost sharing, bundled payments, risk sharing, etc. To ensure cost effectiveness of any contemplated expansion of the horizon scanning system, AHRQ seeks input from industry regarding existing efforts and resources which could impact its feasibility. Information Sought: AHRQ intends to maximize the impact of the taxpayers' investments in research. AHRQ is especially interested in ongoing efforts, existing resources, and expert opinions that can address the following questions related to expansion of AHRQ's healthcare horizon scanning system: 1. What are other examples of types of changes that might impact costs that should be tracked by the horizon scanning system? 2. What existing data sources could be used to track these changes? What methods could be used to identify trends in these data sources? Is the information contained in these sources updated in real time? If not, how often are they updated? 3. What methods could be used to forecast the impact on cost? 4. Are there innovative methods taking advantage of social media and crowdsourcing that could be used? 5. What should be the threshold for an innovation having a high impact on cost? 6. Is the tracking of innovations expected to have a high impact in 1-2 years reasonable? If not, what length of time is more appropriate? 7. Any other issues that respondents feel are relevant. Responses shall be submitted via email to the Contract Specialist identified in this notice and shall be submitted no later than the specified due date in this notice. This notice does not obligate the Government to award a contract or otherwise pay for the information provided in response. The Government reserves the right to use information provided by respondents for any purpose deemed necessary and legally appropriate. Any organization responding to this notice should ensure that its response is complete and sufficiently detailed. Information provided will be used to assess tradeoffs and alternatives available for the potential requirement and may lead to the development of a solicitation. Respondents are advised that the Government is under no obligation to acknowledge receipt of the information received or provide feedback to respondents with respect to any information submitted. All submissions become the property of the Federal Government and will not be returned. Not responding to this RFI does not preclude participation in any future RFP/solicitation. The information provided in this RFI is subject to change and is not binding on the Government. Any solicitation resulting from the analysis of information obtained will be announced to the public in Federal Business Opportunities (www.FBO.gov) in accordance with the FAR Part 5. However, responses to this notice will not be considered adequate responses to a solicitation. Confidentiality: No proprietary, classified, confidential or sensitive information should be included in your response. The Government reserves the right to use any non-proprietary technical information in any resultant solicitation(s).
