- Past Performance Questionnaire (Attachment 3) Wage Determination 2005-3033 Revision 17 (29 Dec 15) (Attachment 2) Performance Work Statement (Oct 2015) - Maintenance, Inspection, and Repair of Overhead Doors, Automatic Doors, and Electric Gates (Attachment 1) Request For Quote - Maintenance, Inspection, and Repair of Overhead Doors, Automatic Doors, and Electric Gates Synopsis: The 1st Special Operations Contracting Squadron, located at 350 Tully Street, Bldg. 90339, Hurlburt Field AFB, FL 32544, intends to make a single award for Overhead Doors and Gates Maintenance and Repair at Hurlburt Field, FL. with a period of performance 1 May 2016 - 30 April 2017. The contractor shall provide management, services, tools, supplies, and equipment, to perform inspections, maintenance and repair services for overhead roll-up doors, electric vehicle gates, specified base-wide gates, and automatic sliding personnel doors at Hurlburt Field, Florida in a manner that will ensure continuous and safe operation of the doors and gates listed in the Appendices. Furthermore, the contractor shall perform the services required in the performance work statement using standard industry practices and shall comply with all environmental laws, Office of Safety and Health Administration (OSHA) standards and applicable federal, state, and local laws. Prior to ordering parts and/or commencing work, the contractor shall submit a detailed estimate to the government representative for approval. The estimate shall include at minimum, parts, labor, shipping and handling, and/or any other costs involved. This acquisition is being offered for competition to eligible 8(a) business concerns. The North American Industry Classification System (NAICS) code for this project is 811310 with a size standard of $7.5M. The Contracting Officer anticipates awarding a firm-fixed price type contract to the successful contractor selected using Past Performance Tradeoff (PPT) procedures. The Government reserves the right to cancel this solicitation, either before or after the closing date. In the event the Government cancels this solicitation, the Government has no obligation to reimburse an offeror for any costs." FAR 52.212-3, Offeror Representations and Certifications - Commercial Items (Nov 2015) applies to this Request for Proposal and the offeror must include a completed copy of this provision with their proposal or indicate (with DUNS Number) that current information is available on the federal government website System for Award Management (SAM) https://www.sam.gov. Prospective contractors must be registered in SAM prior to award of a government contract. Additionally, the offeror shall comply with the DFARS clause at 252.232-7003 Electronic Submission of Payment Requests and Receiving Reports (Jun 2012).
