- Attachment # 5 - DFARS 252.209-7991 Attachment #4 - DOL - Wage and Hour Rates Attachment #3 - Bid Sheet Attachment #2 - Performance Work Statement (PWS) Attachment #1 - Solicitation The purpose of this contract is to obtain clean wiping rags for the maintenance shops throughout the Maine Army National Guard. Contractor shall provide all necessary labor, materials, transportation, planning, administration, and management to assure that all services comply with the contract to pick up soiled wiping rags and furnish clean cotton wiping rags in size to all six locations. The Period of Performance: 1 April 2016 - 31 March 2017 (One Year Term Only - No Options) The United States Property and Fiscal Office for Maine has issued as a Request for Quotation (RFQ) under W912JD-16-T-0002. The Government intends to award a Firm-Fixed Price Contract to the offeror deemed responsible in accordance with the Federal Acquisition Regulation (FAR). It is the Government's intends to award without discussions. The procurement action is 100% Small Business Set-Aside. The North American Industry Classification (NAICS) Code is 812332 - Industrial Launderers. The Size Standard for Small Business is $38.5 Million. DOL - Wage and Hour - 2005-2241 # Rev #18 - dated: 12/29/2015 (5) DFARS - 252.209-7991 - Representation by Corporations Regarding an Unpaid Delinquent Tax Liability or a Felony Conviction under any Federal Law 2016 (OCT 2015) Submit all questions regarding this solicitation must be submitted NO -LATERT THAN 2:00 p.m. Eastern standard time, on Thursday, 18 February 2016, to darlene.r.howes.civ@mail.mil. Vendors are strongly encouraged to submit questions early in the process. Prompt payment discounts will not be considered in the evaluation of offers. However, any offered discount will be part of the award, and will be taken if payment is made within the discount period as indicated in the vendor's offeror.
