- Solicitation Past Performance Reference List Past Performance Questionare Performance Work Statement This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement is being issued as a request for quotation (RFQ) and constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular #2005-80 Effective 02 Mar 2015. This combined synopsis/solicitation is being competed as a Total Small Business Set-Aside. This acquisition will be competed to the full extent required by regulation. The NAICS code that applies is 561720 and business size standard is $18 million. This requirement is for the 148 th Dutch Squadron Janitorial Services located at the 162D Wing, 1500 E Valencia Rd, Tucson, AZ, 85706. This requirement is for one (1) base year plus four (4) option years with a six (6) month extension. Services are required IAW the schedule provided on the Performance Work Statement (see schedule in attached PWS). The contractor shall provide all labor, tools, materials, and transportation necessary to accomplish the janitorial services IAW the attached PWS. This shall cover the period set forth in the contract. Offerors shall forward past performance questionnaires to at least 3 references and request that responses be sent directly to 162D Wing Mission Support Contracting office by 12:00 pm Arizona time 19 February 2016. Offerors are urged to review the submission requirements in FAR Provisions 52.212-1 and its addendum. This is a best value acquisition. Evaluation factors are addressed in FAR Provision 52.212-2 in the solicitation. The Government will make an award to the responsible offeror whose offer conforming to the solicitation is the most advantageous to the government. SITE VISIT: A site visit will be conducted on Tuesday, 16 February at 12:00 p.m. Arizona Time, at the Base Contracting Office, Tucson, Arizona, 85706. Due to limited space, we request no more than three personnel from your organization attend. All attendees must have a Government issued photo ID, such as a valid state driver's license, to gain access to the facility. Information provided at this site visit as well as answers to questions shall not change/qualify the terms and conditions of the Request for Quote and specifications. Terms remain unchanged unless Request for Quote is amended in writing. If an amendment is issued, normal procedure relating to the acknowledgment and receipt of Request for Quote amendments shall apply.
