- blank W-9 MTC Vendor Self-Certification form MTC Terms and Conditions REQUEST FOR QUOTE (RFQ) This is a subcontracting opportunity with Management and Training Corporation (MTC), operator of the Flint Hills Job Corps Center (FHJCC). The bids for the solicitation for psychiatric services are due February 11, 2016 at 4:00 PM (CST). The attached Request For Quote (RFQ) contains the requirements of the solicitation. The blank W-9, MTC terms and conditions, and MTC Vendor Self-Certification forms are provided for your convenience. All forms and references must be returned with our Bid Sheet in order to be deemed responsive. MTC reserves the right to reject any or all bids or waive any formality connected with the bids. MTC will be the sole judge of which bid is the best value and its determination and finding on such decision will be final. MTC may elect to award contract on a per deficiency basis.
