- Schedule Price Listing Statement of Work Clauses Cover Letter The Federal Bureau of Prisons, Field Acquisition Office, and Grand Prairie, Texas intend to issue solicitation a single award RFQP04151600006 for the provision Optometry Services to be performed at the Federal Correctional Institution (FCI) in Seagoville, Texas. The requirement will be for Optometry Services. The contract will consist of a base plus four options year periods which include 480 sessions per year. The contract type will be an indefinite delivery/requirement type contract with firm-fixed unit prices. The applicable North American Industry Classification System (NAICS) code for this requirement is 621320 with a small business size standard of $20.5 million. The solicitation is restricted to small business. All responsible sources may submit a quotation which will be considered. Faith-Based and Community-Based Organizations can submit offers/bids/quotations equally with other organizations for contracts for which they are eligible. The solicitation will be available on or about January 7, 2016 and will be distributed solely through the website at www.fbo.gov. All future information concerning this acquisition, including solicitation amendments will be distributed through the website. The anticipated deadline for receipt of quotations is February 10, 2016 12:00 P.M. Central.
