Description
This is a SOURCES SOUGHT ANNOUNCEMENT ONLY. Responses to this Sources Sought must be in writing. The purpose of this Sources Sought Announcement is for market research to make appropriate acquisition decisions and to gain knowledge of potential qualified Large and Small Businesses interested and capable of performing the work. Responses to this notice shall include the following: (a) company name (b) address (c) point of contact (d) phone, fax, and email (e) DUNS number (f) Type of small business, if applicable, (e.g. Services Disabled Veteran Owned small Business, Veteran-owned small business, 8(a), HUB-Zone, Women Owned Small Business, Small disadvantaged business, or Small Business) and (g) a capability statement that addresses the organization's qualifications and ability to perform as a contractor for the work described below. All Offerors who provide goods or services to the United States Federal Government must be registered in the System for Award Management (SAM) database found at https://www.sam.gov. Registration must include Representations and Certifications. Additionally, all Service Disabled Veteran Owned Businesses or Veteran Owned Businesses who respond to a solicitation on this project must be registered and verified with the Vet-Biz Registry located at http://vip.vetbiz.gov. The U.S. Department of Veterans Affairs (VA) is seeking a contractor to assist with the provision of case management services through the HUD-VASH program. The program is a partnership between the U.S. Department of Housing and Urban Development (HUD) and the VA Supportive Housing (VASH) program to place and maintain homeless Veterans in permanent, community based housing using HUD Section 8 Housing Choice Vouchers that are paired with ongoing VA case management services. Many of these Veterans have co-morbid conditions that include serious mental illnesses, substance abuse disorders, and other serious medical conditions. VA intends to engage a Contractor to provide ongoing case management services to HUD/VASH eligible Veterans and their families. Contractors that are interested in providing these services should be aware that HUD-VASH is a national program serving Veterans and their families throughout the country. This program continues to expand and has become one of the largest VA interventions to assist homeless Veterans. It represents on of VA's most significant efforts to achieve the President's goal of ending homelessness among Veterans. To be eligible for the HUD-VASH program, a Veteran must be: a.Eligible for VA health care as determined by the local VA medical center b.Determined to be homeless based on McKinney-Vento Act definitions by the local VA medical center HUD-VASH program with every effort made to identify and serve the most chronically homeless and vulnerable Veterans. The target population for the HUD-VASH case management contract shall include chronically homeless Veterans who are the most vulnerable and may have severe mental health and/or substance abuse problems. Veterans may not be chronic and may fall into one of the priority groups such as women, women Veterans with children, OEF/OIF/OND Veterans, and/or Veterans who have physical health problems and/or Substance Use Disorders (SUD), with frequent emergency room visits, multiple treatment failures, and limited access to other social supports. Through this contract initiative, the VA will place individuals into housing and will then refer Veterans to the contractor for case management and supportive services; who will assist the Veteran in maintaining housing until the Veteran is eligible for discharge. It is the VA's intention to solicit proposals on a competitive basis for a Firm-Fixed Price contract. Proposed prices shall be all inclusive and will include malpractice insurance, benefits and contractor's fees. The contract will be awarded as a base year (one year from date of award), with four (4) one-year renewal options that can be exercised at the option of the Government. This solicitation will be issued as a commercial item in accordance with FAR Part 12 and FAR Part 15. NAICS is 624190. The solicitation is expected to be issued on or about 02/01/2016 with responses due on or about 02/29/2016.
