Description
Solicitation Attachment #1 Contract Pricebook Catalog is hereby replaced in its entirety by adding document entitled, Attachment #1 Contract Pricebook Catalog(Amendment 0002), attached to this solicitation amendment. SUMMARY OF CHANGES: Solicitation SPE2D1-15-R-0005 is amended in order to incorporate changes: (1.) Solicitation Attachment #1 Contract Pricebook Catalog is hereby replaced in its entirety by adding document entitled, Attachment #1 Contract Pricebook Catalog(Amendment 0002), attached to this solicitation amendment. (a.) Vendors now must provide all commercial part numbers for each configuration. If the same part number applies to both configurations it must be provided under both configurations in the spreadsheet. Zero dollar part numbers must also be provided. (b.) Items that are the same for both the Basic Configuration and Enhanced Configurations must have identical pricing under each configuration. (c.) Vendors shall provide a ‘Long Description' for each offered part number, including all components and part numbers that make up the item. (d.) Bundled or Administrative Part Numbers created solely for the Government are not permissible for offer submission and only part numbers used and sold commercially can be offered. Vendors must provide official commercial catalogs and commercial invoices to substantiate that the same commercial price structure and part numbers are being offered to the Government. (e.) After contract award, administrative part numbers shall be created for ease of ordering, where a customer can order a full configuration by using a single administrative part number. (2.) DLA is providing clarifications and responses to vendor questions that have been received to date. (a.) Can you please specify the exact type and quantity of cables and accessories required to be included in the proposal for the Vital Signs Monitor (VSM)? (i.) Minimum Requirement #6 requires that the VSM monitor and display NIBP for Adult, pediatric, and infants. Should respondents include NIBP cuffs for each patient type? DLA RESPONSE: Yes, respondents should include NIBP cuffs for each patient type. (ii.) Minimum Requirement #11 requires 2 ports for Temperature Monitoring. Should respondents include 2 temperature probes? DLA RESPONSE: Two (2) sensors are required for temperature monitoring per requirement #11. (iii.) Minimum Requirement # 7 requires 2 channels of Invasive Blood Pressure (IBP). Minimum Requirement #15 specifies monitoring and displaying Intracranial Pressure. Can you confirm the minimum requirement for Invasive Pressure Monitoring is 3 simultaneous channels? Should respondents include 3 Invasive pressure cables? DLA RESPONSE: Yes, three (3) channels of invasive pressure are required: - Two (2) are required for minimum requirement #7; and - One (1) is required for minimum requirement #15. (b.) What will be the composition and specialties for inclusion on the Source Selection Authority as referenced in SOW(6.)(j.)? For example, clinical, technical, logistical, etc.? DLA RESPONSE: DLA does not disclose any information regarding the Source Selection Authority (SSA) aside from what initial disclosures have already been made in the Statement of Work and the solicitation. (c.) Will there be an opportunity or requirement for respondents to provide a physical demonstration of the proposed Basic and Enhanced configurations as part of the evaluation process? DLA RESPONSE: There will not be any physical demonstrations for the Basic and Enhanced configurations as part of the evaluation process.
