- Amendment Three (3) issued to cancel the solicitation. Requirement changed and to be rewritten. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The Solicitation Request for Quotation (RFQ) number is IHS1266784. The RFQ and attached document incorporate provisions and clauses in effect through Federal Acquisition Circular FAR 2005-83/07/02/2015. Full and Open Competition. NAICS 339114, size standard 500 employees. Line Items are shown on attached Quote Schedule. Purpose: Procure various toothbrushes, toothpaste and dental floss for IHS beneficiaries. The Indian Health Service (IHS) National Supply Service Center intends to award a fixed-price IDIQ requirements purchase order(s) in response to the RFQ. The performance period is one year from date of award. Orders will be placed on an as needed basis. The government reserves the right to make one or multiple awards. Date and Place of Delivery: Delivery is required 5-10 days after each order placement. Deliver to: IHS National Supply Service Center, 501 NE 122 Street, Suite F, Oklahoma City, OK 73114. FOB Destination. FAR 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition and includes the following: Evaluation Factors: 1. Delivery timeframe 2. Product attributes (i.e. whitening, waxed/flavored floss, ergonomical, etc.) based on specifications and product samples review 3. Variety of products (colors, characters, etc.) 4. Warranty Terms 5. Price* 6. Provision of value added items *Price is secondary to technical / non-price factors stated above. The Government will award a purchase order(s), resulting from this solicitation, to the responsible quoter whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. 52.212-4 Contract Terms and Conditions-Commercial Items applies to the RFQ. No addenda. 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items applies to the RFQ with clauses: 52.203-6, 52.204-10, 52.209-6, 52.222-3, 52.222-19, 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.222-50, 52-223-18, 52.225-3 with Alt. ADDITIONAL REQUIREMENTS: QUOTE SUBMISSION INSTRUCTIONS: All responsible offerors may submit a quote, which shall be considered by the Agency. Quotes with requested product samples and information shall be submitted to the IHS National Supply Service Center, Attn: Sara Marcus, 501 NE 122 Street, Suite F, Oklahoma City OK 73114 no later than 5:00 p.m. Central Standard Time, July 31, 2015. The quote must be submitted in package showing the RFQ number, your company name and address and to the attention of Sara Marcus. To be considered for award, please complete and sign/provide the following: 1. Completed Quote Schedule or an Itemized Price Quotation that includes information required in the RFQ Quote Schedule. 2. State the delivery timeframe for orders. If lead time is required you must identify lead time and delivery time separately. 3. Item Specifications and warranty information for products quoted. 4. Quote samples. For each model toothbrush, toothpaste and floss quoted provide 1 sample. Samples must be provided and packaged in same manner and size as when there are given to patients. 5. Identify value added items included, i.e. patient sample bags, trial size mouth rinse and quantity discounts. Value added items shall include descriptions, specifications, samples, warranty and prices as applicable. 6. Past performance references: List of 3 current customers for which you have provided same or similar products. Provide customer name and contact information. DPAS: Not Applicable RFQ questions must be submitted by email to sara.marcus@ihs.gov no later than July 24, 2015, 5:00 p.m. CST. Questions will be answered by amendment to the RFQ. Telephone questions will not be accepted. Can you give me an idea of the usage on this also the size of the toothpaste? One year usage is reflected in the quote schedule which was included in the solicitation posting. 2. Can you confirm whether or not this contract will be awarded all or none? This will not be known until quotes are in and evaluated. 3. May I please have the previous bid tabs on these items so I may bid competitively? Previous bid information is not released during the solicitation period to ensure the requirement is not compromised. AMENDMENT TWO TO SOLICITATION IS ISSUED TO EXTEND THE RFQ CLOSING DATE TO AUGUST 5, 2015, 5:00PM CENTRAL TIME. AMENDMENT THREE CANCELS THE SOLICITATION.
