STATEMENT OF WORK A.GENERAL INFORMATION B. 1. Title of Project: Concrete Platform at VAMC, Northport, 79 Middleville Road, Northport, NY, 11768. 632-15-3-5303-0162. 2. Scope of Work: Contractor shall supply all labor, tools, materials, equipment, supplies and supervision to perform all work for the installation of a concrete platform at the Department of Veterans Affairs Medical Center, 79 Middleville Rd. Northport, New York 11768. The foundation shall be forty-five (45) feet by forty-five (45) feet by 6" thick. Concrete shall be 4000psi reinforced concrete. There shall be #5 rebar and 6"x6" #6 welded wire mesh throughout. Footings shall be to a depth of four (4) foot below grade. The slab shall be pitched for drainage.contractor shall submit shop drawings showing rebar lay-out, lengths, and fastening methods. Construction methods and workmanship shall comply with manufacturer's specifications and general accepted trade practices. Materials to be installed in permanent work shall be approved by the Government prior to installation. Contractor shall remove, cut, alter, replace, patched and repair existing work as necessary to install new work. Upon completion of contract contractor shall deliver work complete and undamaged. Construction methods and workmanship shall comply with VA requirements; see http://www.cfm.va.gov/til/spec.asp Materials and equipment accruing from any work, or part thereof shall become property of the contractor and shall be removed by the contractor from the Medical Center, however any items that are recyclable (steel, etc.) shall be properly disposed by recycling and shall be a credit to the VA government; proof of recycling with waste manifest shall be provided to the VA Contracting Officer. Prior to commencing work, general contractor shall provide proof of OSHA certified "competent person" with a 30-Hour safety certificate (cp)(29CFR 1926.20 (b) and shall maintain a presence at the work site whenever the general or sub-contractors are present and all other employees shall have a 10-Hour safety certificate issued by OSHA or other relevant competency training as determined by the D.V.A.M.C. All tools, materials, and personal property shall be secured at all times. The contractor shall be responsible for safety precautions, including warning signs and barricades during the erection of the Pre-Engineered building and have adequate personnel on hand to insure a safe working environment. The contractor is responsible for all debris and its removal from Government property. Work site shall be cleaned daily. All meetings, inspections and or work schedules must be coordinated and pre-approved prior to commencing by D.V.A.M.C. Representative. The contractor shall examine all of the work as specified to determine the full scope of work prior to any estimate given to the D.V.A.M.C. The proposal shall be in writing, original and one (1) copy. An itemized breakdown (i.e. number of hours by trade, task, and material) and each of the trade/areas listed as separate line items. The work includes but not limited to: all equipment, materials, and labor for earth work including excavation, fill, backfill, and site restoration and cast-in-place structural concrete for the foundation and slab. All work shall be performed in a safe and reliable manner. All work shall be performed as described herein and as specified in the body of the contract documents and conforming to all local, state, federal, and D.V.A.M.C. codes and regulations. All work to be performed at the Department of Veterans Affairs Medical Center 79 Middleville Rd. Northport, New York 11768 It should be noted that this contract does not involve access to VA sensitive information and does not involve access and/or connection to the VA information system. 3. Performance Period: The contractor shall commence work within fourteen (14) days following the contract award. Work shall complete within thirty (30) days of commencement unless delayed by unforeseen circumstances. 4. Type of Contract: Firm-Fixed-Price. 5. Evaluation Criteria: Lowest Price-Technically Acceptable, price factor. 6. Place of Performance: VA Medical Center, 79 Middleville Road, Northport, NY 11768. ---END---
