Description

This is a REQUEST FOR INFORMATION notice for market research purposes only to determine the availability of qualified sources capable of providing the services described below. Potential contractors are invited to provide a response via e-mail to Scott Reed at scott.reed2@va.gov by 12:00 PM PST on October 8, 2015. Responses will be used to determine the appropriate acquisition strategy for a potential future acquisition. Please clearly identify any information your company considers sensitive or proprietary. Service: Community Based Outpatient Clinic (CBOC) for the Department of Veterans Affairs (VA), in the location of Sandpoint, Bonner County, Idaho. The proposed CBOC services include leasing, furnishing, equipping, and staffing one CBOC and providing healthcare services to veterans. The proposed CBOC will provide services for an estimated quantity of 2,000 veteran patients. The current CBOC is operated by a contractor in Sandpoint, Bonner County, Idaho. The anticipated contract will be a re-competition for services to begin at the end of that contract, and the anticipated contract period is a base year plus four (4) one-year options, with services estimated to commence on October 1, 2016. Interested potential contractors: Please provide the following information. 1) Company name, address, point of contact name, phone number, e-mail address, and DUNS #. 2) The North American Industry Classification System (NAICS) code is 621498, All Other Outpatient Care Centers; the largest a firm can be and still qualify as a small business for Federal Government programs under NAICS code 621498 is no larger than $20.5 million average annual receipts. Please state whether or not your company has experience setting up and operating a VA CBOC and whether or not you have experience with current PACT (Patient Aligned Care Team) modeled clinics. The capabilities statement request is not a Request for Quotations, Request for Proposals or Invitation for Bids, nor does it restrict the Government to an ultimate acquisition approach, but rather the Government is requesting a short statement regarding your company's ability to provide the services listed above. Any commercial brochures or currently existing marketing material may also be submitted with the capabilities statement. Submission of capabilities statement will assist our office in tailoring the requirement to be consistent with industry standards. The capabilities will be evaluated for the purpose of determining to Set-Aside for the Small Business Community or to conduct as an Unrestricted Procurement. 6) Provide any additional feedback that you feel is relevant (e.g., problems or any other Issues experience with similar contracts). You may also submit comments about the DRAFT PWS. This posting is for information and planning purposes only and is not to be construed as a commitment by the Government. The Government will not pay for information solicited. Respondents will not be notified of the results of the market research. The DRAFT Performance Work Statement (PWS) follows.
