- Complete RFQ, including SF18, Statement of Need, Provisions, and Clauses. Offeror must complete and include with their quote the provision for Representation by Corporations Regarding an Unpaid Delinquent Tax Liability or a Felony Conviction Under Any Federal Law (Class Deviation) (March 2015), as well as provision FAR 52.212-3 Offeror Representations and Certifications—Commercial Items (MAR 2015) Provisions, standalone. Offeror must complete and include with their quote the provision for Representation by Corporations Regarding an Unpaid Delinquent Tax Liability or a Felony Conviction Under Any Federal Law (Class Deviation) (March 2015), as well as provision FAR 52.212-3 Offeror Representations and Certifications—Commercial Items (MAR 2015) Clauses, standalone Statement of Need, standalone COMBINED SYNOPSIS/SOLICITATION ANALOG AND DIGITAL RADIO TEST SETS, BRAND NAME OR EQUAL TO AEROFLEX 8800S, WITH TRADE-INS All responsible sources may submit a quotation which shall be considered by the agency. (i)This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice and in accordance with the simplified acquisition procedures authorized in FAR Part 13. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii)This solicitation is issued as a request for quotation (RFQ). Submit written quotes on RFQ number WE-133W-15-RQ-1119. (iii)The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-84. (iv)This solicitation is being issued as a Total Small Business Set-Aside. The associated NAICS code is 334515. The small business size standard is 500 employees. (v)The following are the intended contract line item numbers, items, quantities, and units of measure: Line Item Number: 0001; Analog and Digital Radio Test Sets, brand name or equal to the Aeroflex 8800S, to include trade-in of seven used Aeroflex COM-120B Communications Service Monitors, in accordance with the Statement of Need; Quantity: 7; Unit of Measure: Each (vi)See the included Statement of Need for a complete description of the requirements for the items to be acquired. (vii)Delivery will be within 6-8 weeks of contract award, to the following address: National Weather Service, 125 South State Street, Rm 1311, Salt Lake City, UT 84138. FOB: Destination (viii)FAR 52.212-1 Instructions to Offerors-Commercial Items (Apr 2014), applies to this acquisition. NOTICE TO OFFERORS: Instructions for submitting quotations under this request for quote must be followed. Failure to provide all information to aid in the evaluation may be considered non-responsive. Offers that are non-responsive may be excluded from further evaluation and rejected without further notification to the offeror. Quotes shall be fully executed and may be provided on the Standard Form (SF) 18. If the SF 18 is not used, the quote shall include all details as requested on the provided SF 18, including completion of the fillable provisions, along with a statement that all provisions and clauses referenced in the solicitation are agreed to. Any acknowledgements of solicitation amendments will be provided on the SF 30 (provided at the time a solicitation amendment is created). (ix)FAR 52.212-2 Evaluation-Commercial Items (OCT 2014), applies to this acquisition. (x)FAR 52.212-3 Offeror Representations and Certifications-Commercial Items (MAR 2015), applies to this acquisition and a completed copy of the provision must be included with the offer. The full text of the provision is included in the attached SF18 Request for Quotation. (xi)FAR 52.212-4 Contract Terms and Conditions-Commercial Items (MAY 2015), applies to this acquisition. (xii)FAR 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items (MAY 2015), applies to this acquisition. (xiii)Additional contract provisions and clauses applicable to this acquisition are provided by reference or in full text in the attached SF18 Request for Quotation. (xiv)The Defense Priorities and Allocations System (DPAS) and assigned rating does not apply to this acquisition.
